单纯心肌舒张功能障碍与多种心肌炎症因子的相关性研究

摘　　要：目的测定心肌舒张功能不全(DHF)患者多种血清炎症因子的水平变化,同时探讨各项超声心动学指标与各炎症因子之间的相关性,了解心肌舒张功能障碍的发病机制以及其与炎症反应的关系。方法选取来我院就诊的门诊和病房患者中,确诊为心肌舒张功能不全的患者(不包括收缩功能不全)组成研究组(635例)和健康志愿者(397例)组成的对照组,记录接诊时基线信息,并记录多个超声心动图指标,同时检测空腹血浆的NT-proBNP、高敏C反应蛋白(hs-CRP)、白介素-6(IL-6)、肿瘤坏死因子α(TNF-α)、单核细胞趋化因子-1(MCP-1)的浓度。结果 5项炎症因子的在DHF时均明显升高,NT-proBNP、MCP-1、TNF-α显示了较高的AUC以及敏感性、特异性、阳性预测值、阴性预测值;不同年龄组的IL-6和MCP-1无差异;按Ganau的左室重构分型的组别,hsCRP和TNF-α无差异。结论 DHF与炎症反应存在显著相关性;NT-proBNP、MCP-1、TNF-α具有较高的DHF的诊断价值。Objective To determine cardiac diastolic dysfunction （DHF） in patients with a variety of changes in serum levels of inflammatory cytokines, and explore various echocardiographic markers of the correlation between inflammatory cytokines, designed from a deeper understanding of cardiac diastolic function the palhogenesis of disorders as well as its relationship with the inflammatory response. Methods I outpatient and ward patients, diagnosed as diastolic dysfunction （excluding systolic dysfunction） study group composed of （635 cases） and healthy volunteers （397 cases） consisting of the control group baseline information when recording admissions and records multiple eehocardiographic indices, while fasting plasma NT-proBNP, high-sensitivity C-reactive protein （hs-CRP）, interleukin -6 （IL-6） . tumor necrosis factor-α （TNF-α）, monoeyte chemotactic factor -1 （MCP-1 ） concentrations. Results 5 of inflammatory cytokines were significantly increased when the DHF, NT-proBNP, MCP-1, TNF-α showed higher AUC and the sensitivity, specificity, positive predictive value, negative predictive value; different age groups of IL-6 and MCP-1 was no difference; Ganau left ventricular remodeling by genotyping group, hs-CRP and TNF-α was no difference. Conclusion there was a significant correlation with DHF and myocardial inflammatory reaction ; NT-proBNP, MCP-1, TNF-α has a high diagnostic value of DHF.
