隧道式拖线术治疗低位复杂性肛瘘的多中心临床研究

摘　　要：目的:评价隧道式拖线术治疗低位复杂性肛瘘的疗效。方法:采用多中心随机对照研究方法,纳入90例低位复杂性肛瘘患者,随机分为治疗组和对照组,每组45例。治疗组采取隧道式拖线术治疗,对照组采用切开术治疗。比较两组患者术后的主要临床症状、体征,肛门功能,及瘢痕面积、创面长距、创面宽距、创面愈合时间、住院费用。结果:术后治疗组患者在疼痛、出血、渗出物、肛缘肿胀、排便方面优于对照组(P<0.05)。治疗组患者的瘢痕面积、创面长距、创面宽距均小于对照组(P<0.01),愈合时间较对照组明显缩短(P<0.01),住院费用较对照组明显减少(P<0.01)。结论:隧道式拖线术治疗低位复杂性肛瘘能减轻患者术后疼痛,减少出血及创面渗出,缩短创面愈合时间,降低患者住院费用,减小术后瘢痕,有效地缓解肛门坠胀症状。Objective： To evaluate the efficacy of thread-dragging through fistula method in treating patients with low-complex anal fistula. Methods： A multieenter randomized controlled study was made, and 90 patients with low-complex anal fistula were randomly divided into treatment group and control group with 45 cases in each group. The treatment group was treated by thread-dragging through fistula method, and the control group was treated by incision method. The clinical symptoms and signs, anal function, scar size, wound length and width, wound healing time, and hospitalization expenses after operation were compared. Results： After operation, the treatment group was significantly better than the control group in the items of pain, bleeding, exudate, anal swelling and defecation （ P 〈 0.05 ）. The sear size, wound length and wound width of treatment group were lower than those of control group （ P 〈 0.01 ）. The wound healing time of treatment group was obviously shortened （P 〈0.01 ） and the hospitalization expense of treatment group was remarkably reduced （P 〈 0.01 ） compared with control group. Conclusion： The method of thread-dragging through fistula in treating low-complex anal fistula can reduce pain, bleeding, exudate and scar size, shorten the wound healing time, and decrease the hospitalization expenses.
