摘  要:目的:研究凝血系统有关指标在骨科大手术患者手术前后的临床表现。方法:选取2011年4月至2012年3月期间,我院相关科室所收治的骨科大手术患者78例为研究对象,分别对其术前24 h与术后24、72 h的FIB(纤维蛋白原)、TT(凝血酶时间)、PT(凝血酶原时间)、APTT(活化部分凝血活酶时间)、PLC(血小板计数)等凝血系统相关指标进行测定。结果:深静脉血栓(DVT)形成16例,术前24 h与术后24、72 h的FIB、APTT、PLC检测结果差异较大,具有统计学意义(P<0.05)。比较DVT组与非DVT组患者发现,术前FIB与APTT指标差异不明显,不具有统计学意义(P>0.05),但术后24、72 h差异较大,具有统计学意义(P<0.05)。结论:FIB、APTT、PLC凝血系统指标可作为骨科大手术患者手术前后DVT形成的诊断依据,为临床诊断DVT形成提供一定的参考依据。Objective To analyze the changes of coagulation system related indicators in patients undergoing major orthopedic surgery. Methods 78 patients undergoing major orthopedic surgery from April 2011 to March 2012 in the relevant departments of the hospital were included. Level of fibrinogen (FIB) and thrombin time (Tr), prothrombin time (PT), activated partial thromboplastin time (APTY) and platelet count (PLC) of patients 24 h before the surgery and 24 h and 72 h after the surgery were detected. Results Deep vein thrombosis (DVT) was occurred in 16 patients, there were significant differences of FIB, APTr and PLC between 24 h before the surgery and 24 h, 72 h after the surgery (P 〈 0.05). Before the surgery, there was no significant difference of FIB and APTT between DVT group and non-DVT group (P 〉 0.05), but at 24 h and 72 h after the surgery, there were significant differences (P 〈 0.05). Conclusion FIB, APTr, PLC could be used as indicators of the coagulation system for the diagnosis of DVT in patients undergoing major orthopedic surgery.
