Bifrontal-temporal decompressive craniectomy under coronal incision combined with intracranial pressure monitoring for patients with bifrontal brain contusion and laceration

摘  要:目的:探讨冠状切口双额颞去骨瓣减压术和颅内压监护治疗双额叶脑挫裂伤的手术时机和手术方式。方法:回顾性分析52例双额去骨瓣减压术治疗双额叶脑挫裂伤的病例,探讨手术时机和手术方式。探讨颅内压监测的意义。结果:两组52例手术患者随访6个月,按GOS评分评定预后,监测组恢复良好19例,中残2例,重残1例,植物生存1例,死亡3例。对照组恢复良好17例,中残2例,重残2例,植物生存2例,死亡3例。结论:双额叶脑挫裂伤患者及时行冠状切口双额颞去骨瓣减压术可取得较好的临床效果。应用颅内压监测能动态地观察颅内压力变化,提供客观准确的颅内压,改善患者预后。Objective To explore the operation timing and manner of bifrontal-temporal decompressive craniectomy under coronal incision combined with intracranial pressure monitoring for patients with bifrontal brain contusion and laceration. Methods Clinical data of 52 patients with bifrontal brain contusion and laceration receiving bifrontal-temporal decompressive craniectomy under coronal incision combined with intracranial pressure monitoring were retrospectively analyzed. The operation timing and manner, and the significance of intracranial pressure monitoring were investigated. Results Fifty-two patients were followed-up for 6 months. According to GOS assessment, 3 cases died, 1 case with persistent vegetative state, 1 cases with severe disability, 2 cases with moderate disability, 19 cases got good recovery in monitoring group; 3 cases died, 2 case with persistent vegetative state, 2 cases with severe disability, 2 cases with moderate disability, 17 cases got good recovery in control group. Conclusion The bifrontal-temporal decompressive craniectomy under coronal incision was an effective method for patients with bifrontal brain contusion and laceration. The intracranial pressure monitoring could dynamically and objectively reflect the intracranial pressure.
