摘　　要：目的:比较Narcotrend监测下靶控输注丙泊酚全凭静脉麻醉(TIVA)与静吸复合麻醉的临床麻醉效果。方法:80例行腹腔镜下子宫肌瘤剔除术患者随机分为两组:丙泊酚TIVA组(A组,n=40)和静吸复合麻醉组(B组,n=40)。比较两组血压与心率变化、术中Nacotrend指数(NI)、术后麻醉恢复时间及术后不良反应发生率。结果:B组SBP,DBP,HR在切皮时、切皮后5 min、拔管时与A组比较差异有统计学意义(P<0.05);两组在诱导后2 min及手术结束时NI比较差异有统计学意义(P<0.05),麻醉恢复时间A组显著短于B组(P<0.05);两组患者术后不良反应发生率差异无显著性(P>0.05)。结论:Narcotrend监测下靶控输注丙泊酚TIVA较静吸复合麻醉血流动力学更稳定,术中麻醉深度合适,术后恢复时间更短。Objective To compare the effects of total intravenous anesthesia （TIVA） with target controlled infusion of propofol and intravenous-inhalation anesthesia under Narcotrend monitor. Methods A total of 80 patients scheduled for laparoscopic myomectomy were randomized into 2 groups to receive target controlled infusion with propofol （Group A, n = 40） or intravenous-inhalation anesthesia （Group B, n = 40）. The hemodynamic changes, Narcotrend Index （NI）, anesthesia recovery and adverse reactions after operation were compared. Results The difference of SBP, DBP and HR at skin incision, 5 minutes after skin incision, and the time of extubation between Group A and B were significant （P 〈 0.05）. The NI at 2min after induction and the end of operation between the two groups were significantly different （P 〈 0.05）. The anesthesia recovery time in Group A was significantly shorter than that in Group B （P 〈 0.05）. There was no significant difference of the incidence of adverse reactions between the two groups （P 〉 0.05）. Conclusions Compared with intravenous-inhalation combined anesthesia, hemodynamic and anesthesia depth was more stable and anesthesia recovery time was shorter in total intravenous anesthesia with target controlled infusion of propofol.
