黄芪注射液促进痔瘘创面修复的临床研究

摘　　要：目的 :探讨黄芪注射液促进痔瘘术后创面修复的作用机理 ,拓宽中药制剂的临床应用范围。方法 :治疗组 70例于术后当日除常规治疗方法外 ,同时加用黄芪注射液 2 0ml,静脉滴注 7～ 10天 ,对照组只用常规治疗方法。术后观察患者创面愈合时间 ,并于术后第 3、 7天分别取创面肉芽组织小块作病理切片 ,用光学显微镜、免疫组化以及CMIAS— 0 0 8B病理图文分析系统等方法对其进行观察分析。结果 :临床观察发现 :创面平均愈合时间 ,治疗组与对照组相比具有极显著性差异 (P <0 0 1) ;组织学观察发现 :治疗组创面第 3、 7天毛细血管数、成纤维细胞数以及巨噬细胞相对含量与对照组相比具有极显著性差异 (P <0 0 1)。结论 :黄芪注射液对痔瘘术后创面修复有促进作用 ,可加速毛细血管再生、补充伤口修复所必需的营养物质和微量元素、成纤维细胞的增殖、激活巨噬细胞等几个方面。提示对其他外科创伤修复亦有积极意义。To explore the mechanisms of Huangqi injection in accelerating postoperative wound repair of hemorrhoids and fistula.Methods:140 patients with hemorrhoids or fistula were randomly divided into 2 groups.The 70 patients were treated by Huangqi injection 20ml(containing 40g Huangqi)in 10% glucose 250ml iv drip for 7—days besides routine therapy as Huangqi group while the 70 patients were treated with routine therapy as control group.To observe the postoperative wound healing time,choosing 20 patients form each group,their granulation tissue of wound was made into pathological sections on the 3、7 day after operation,and analyzed the sections with pathology and immunohistochemistry.Results:1.Clinical observation showed that the difference of mean wound healing time between 2 groups is of great significance ( P <0 01).2.Histological observation showed that there was a greatly significant difference of the number of capillaries、fibroblasts and macrophages between 2 groups on the 3、7 day ( P <0 01).Conclusion:Huangqi injection can accelerate the wound repair of hemorrhoids and fistula after operation,which may be thought the following mechanisms 1.accelerating the regeneration of capillary.2.Supplying nutrition and trace elements which are essential in wound repair.3.The proliferation of fibroblast.4.Activating macrophage etc.and may also be good for other wounds repair.
