摘 要:目的探讨胰岛素样生长因子结合蛋白3(insulin—like growth factor binding protein-3,IGFBP-3)基因-202A/C多态性与膀胱移行细胞癌危险性的关系。方法本研究为以中国四川人群为研究对象的病例对照研究,选取98例膀胱癌患者为病例组,150例性别、年龄相匹配的健康者为对照组。应用聚合酶链反应-限制性片段长度多态性分析方法对单核苷酸多态性(rs2854744)进行基因分型。结果病例组和对照组IGFBP-3-202A/C位点基因型及等位基因差异有统计学意义(X2=12.965,P=0.002;X2=11.545,P=0.001)。携带AA基因型发生膀胱癌的风险是cc基因型的0.224倍(OR=0.244,95%CI=0.109—0.549,P:0.001),携带等位基因A是等位基因C的0.531倍(0R=0.531,95%CI=0.368—0.766,P=0.001)。基因型AA和AC较基因型CC发生浸润性膀胱癌风险显著降低(D尺=0.211,95%CI=0.081~0.548,P=0.001;OR=0.150,95%CI=0.036~0.631,P=0.010),携带等位基因A发生浸润性膀胱癌风险是携带等位基因C的0.363倍(OR=0.363,95%CI=0.191~0.691,P=0.002)。结论IGFBP-3-202A/C多态性可能是膀胱癌的危险因素。Objective To investigate the relationship between polymorphism (rs2854744) at - 202A/C in the insulin-like growth factor (IGF)-binding protein-3 (IGFBP-3) gene and the risk of bladder cancer. Methods In a hospital based case-control study for the Chinese population in Sichuan, 98 patients with bladder cancer and 150 age- and sex- matched healthy subjects were enrolled. One single nucleotide polymorphism (rs2854744) was genotyped by polymerase chain reaction-restriction fragment length polymorphism (PCR-RLFP) technique. Results We found statistically significant differences in the genotypic distribution and allele frequency of IGFBP-3-202A/C between the cases and the control subjects ( X2 = 12. 965, P = 0. 002 ; X2 = I 1. 545, P = 0.001 ). We detected significantly decreased risk of bladder cancer associated with AA genotype ( OR = 0. 244,95% CI = 0. 109 - 0. 549 ; P = 0. 001 ) compared with the CC genotype. Compared to the individual carrying allele C, the risk of bladder cancer with allele A decreased 0. 531 times ( OR = 0.531,95% CI = 0.368 - 0. 766 ,P = 0.001 ). We detected significantly decreased risk of invasive bladder cancer associated with AA and AC genotype (OR = 0.211,95% CI=0.081 -0.548,P =0.001;OR =0. 150,95% CI=0.036-0.631,P=0.010) compared with CC genotype. Compared to the individual carrying allele C, the risk of invasive bladder cancer with allele A decreased 0. 363 times ( OR = 0. 363,95% CI = 0. 191 - 0. 691, P = 0. 002).Conclusion IGFBP-3-202A/C gene polymorphism may be a risk factor for bladder cancer.
