老年与中青年甲状腺切除手术的临床疗效和安全性分析

摘　　要：目的探讨老年与中青年患者行甲状腺切除术的临床疗效与手术安全性。方法选取解放军第四五五医院2010年3月～2012年3月收治的拟行甲状腺切除术患者共142例,根据患者年龄分为老年组(n=65)与中青年组(n=77),观察两组患者手术时间、术中出血量、住院时间及并发症和术后复发情况。结果①中青年组患者手术时间和术中出血量分别为(91.55±10.56)min和(45.81±11.85)mL,老年组患者手术时间和术中出血量分别为(95.22±11.62)min和(42.01±12.33)mL,两组比较差异无统计学意义(P>0.05);中青年组患者住院时间为(5.99±2.61)d,明显少于老年组患者[(10.85±2.52)d],两组比较差异有统计学意义(P<0.05)。②中青年组术后并发症发生率为7.69%(5/65),而老年组并发症的发生率为11.69%(9/77),两组比较差异无统计学意义(P>0.05)。③老年组患者术后复发率为6.15%,中青年组术后复发率为6.49%,差异无统计学意义(P>0.05)。结论老年患者行甲状腺切除术安全可行,具有较好的临床治疗效果,应严格掌握手术适应证,选择合适的手术方式治疗。Objective To investigate the clinical efficacy and the surgical safety of elderly and middle-aged patients underwent thyroidectomy.Methods 142 cases underwent thyroidectomy were admitted in the 455th Hospital of People＇s Liberation Army in March 2010 to March 2012,all the patients were divided into old age group （n =65） and middle-aged group （n =77） according to the patient＇s age; operative time,blood loss,duration of hospitalization and the complications and recurrence were observed in the two groups.Results ①Operative time,blood loss of middleaged group were （91.55±10.56） min and （45.81±11.85） mL,operative time,blood loss of old age group were （95.22±11.62） min and （42.01±12.33） mL,the differences were not statistically significant （P ＞ 0.05）.Duration of hospitalization of middle-aged group were （5.99±2.61） d,duration of hospitalization of old age group were （10.85±2.52） d,the difference was statistically significant （P ＜ 0.05）.②Occurrence rate of complication of middle-aged group was 7.69％（5/65）,occurrence rate of complication of old age group was 11.69％ （9/77）,the difference was statistically significant （P ＜ 0.05）.③Postoperative recurrence rate of old age group was 6.15％,postoperative recurrence rate of middle-aged group was 6.49％,the difference was not statistically significant （P ＞ 0.05）.Conclusion Elderly patients underwent thyroidectomy is feasible and safe,with a better clinical effect,it is right to grasp the operation indications,choose the right operation method.
