经腹高频超声诊断宫内异常早期妊娠的临床价值

摘　　要：目的探讨经腹高频超声诊断宫内异常早期妊娠的临床价值。方法选取东莞市横沥人民医院2010年2月～2012年8月进行超声检查的孕妇,先用常规超声检查,选择妊娠囊或胚胎距皮下<70 mm孕妇,用高频超声检查,高频超声诊断为异常妊娠的孕妇共1653例纳入本研究。在1653例孕妇中随机选择241例孕妇做阴道超声检查。结果本研究诊断结果分别为:①先兆流产:高频超声诊断957例,常规超声诊断728例;②难免流产:高频超声诊断78例,常规超声诊断75例;③不全流产:高频超声诊192例,常规超声诊断158例;④胚胎停止发育:高频超声诊断398例,常规超声诊断285例;⑤葡萄胎:高频超声诊断28例,常规超声诊断25例。241例阴道超声检查的孕妇,均与高频超声诊断相同。结论经腹高频超声与常规超声比较,可以显著提高宫内异常早期妊娠的诊断率,与阴道超声比较,诊断率相当,但高频超声操作更方便,更易被孕妇接受。Objective To investigate the clinical value of abnormal early intrauterine pregnancy diagnosed by abdominal high-frequency ultrasound.Methods The pregnant women checked by ultrasound from February 2010 to August 2012 in Hengli People＇s Hospital of Dongguan City were chosen,they were firstly examined by routine ultrasound.Pregnant women with gestational sac or embryos from subcutaneous ＜ 70 mm were selected,and then checked by high frequency ultrasound,Total 1653 cases abnormal pregnancy were brought into this study.And 241 pregrant women were selected from 1653 cases to do transvaginal ultrasound inspection.Results Diagnosis results of this study were as followed：①threatened abortion：957 cases of high frequency ultrasound diagnosis,728 cases of conventional ultrasound diagnosis; ②inevitable abortion：78 cases of high frequency ultrasound diagnosis,75 cases of conventional ultrasound diagnosis; ③incomplete abortion：192 cases of high frequency ultrasound diagnosis,158 cases of conventional ultrasound diagnosis; ④embryonic development stops：398 cases of high frequency ultrasound diagnosis,285 cases of conventional ultrasound diagnosis; ⑤hydatidiform mole：28 cases of high frequency ultrasound diagnosis,25 cases of conventional ultrasound diagnosis; the diagnosis of 241 cases pregnant women inspected by transvaginal ultrasound were the same with the high frequency ultrasound diagnosis.Conclusion Comparing abdominal high-frequency ultrasound with conventional ultrasound,it can evidently improve the diagnosis rate of abnormal early intrauterine pregnancy.While compare it with vaginal ultrasound,diagnosis rate is almost the same,but it is more convenient and acceptable to do the high-frequency ultrasound operation for pregnant women.
