利用多普勒成像联合高频超声定性诊断体表软组织肿块的临床意义

摘　　要：目的探讨多普勒成像联合高频超声定性诊断体表软组织肿块的临床意义。方法对2012年2～12月来句容市人民医院经手术证实为体表软组织肿块的60例患者采用直接探查法,在病变部位纵、横向十字交叉处观察肿块的发生部位、内部回声、形态、边界、Adler半定量分级等,并以多普勒成像观察肿块内外血流情况,脉冲测定肿块内最大血流速度。后与手术病理诊断对照。两种诊断方法的一致性比较利用Kappa检验。结果多普勒成像联合高频超声诊断对60例体表软组织肿块病变检出率为100.0%。主要软组织肿块超声声像图特点显示,多普勒成像联合高频超声诊断的二维位置、形态、内部回声、边界及分级均符合病理学特征。60例体表软组织肿块38例(63.3%)有彩色血流显示,丰富者14例,散在者24例,平均最大流速为(12.30±6.12)cm/s,平均最小流速为(2.4±0.6)cm/s。结论多普勒成像联合高频超声对定性诊断体表软组织肿块具有较高的临床应用价值,有助于鉴别诊断体表肿物良、恶性,且该方面迅速、安全,无严格的禁忌证,可作为首选的影像检查手段。Objective To explore the clinical significance of using Doppler imaging combined with high frequency ultrasound on qualitative diagnosis of body surface mass lesion.Methods 60 patients from February to December 2012 in the People＇s Hospital of Jurong City who confirmed by surgery for body surface were given soft tissue mass with direct detection method,the lesion site location of bump,boundary,internal echo,form,Adler semi-quantitative grading,etc were observed in the longitudinal and transverse cross intersections,and the Doppler blood flow imaging was used to observe bump inside and outside circumstance,pulse was used to measure the maximum blood flow velocity.Then surgical pathology diagnosis was compared.Kappa test was used to diagnose the consistency of two methods.Results The detection rate of Doppler imaging combined with high frequency ultrasound of 60 cases of skin soft tissue mass lesion was 100.0％.Main mass lesion ultrasonic ultrasonographic characteristics showed that Doppler imaging combined with high frequency ultrasound in diagnosis of two-dimensional position,shape,internal echo,the boundary and grading were conform to the pathological features.Among 60 cases of body surface mass lesion,38 cases （63.3％）showed color Doppler,in which 14 cases were rich,24 cases were scattered,the maximum average velocity was （12.30±6.12） cm/s,the minimum average velocity was （2.4±0.6） cm/s.Conclusion Combined with high frequency ultrasound Doppler imaging of body surface mass lesion has high clinical value in diagnosing,and it is helpful to differential diagnosis of benign and malignant skin lesions,and it is fast,safe with no contraindications strictly,which can be used as the preferred imaging method.
