某院门诊大金额处方用药情况分析

摘　　要：目的了解某院门诊大金额处方的药物使用情况,促进临床合理用药。方法采用回顾性调查研究方法,随机抽查2010年某院门诊西药房1200张300元以上的大金额处方,根据医院处方点评制度相关规定对大金额处方患者情况、处方诊断、处方药品及处方合理性情况进行统计分析。结果该院门诊大金额处方男女患者数基本相等,其中>50岁患者数最多,占67.75%,处方平均金额为424.95元,处方平均用药品种数为3.55种,抗菌药的使用率为26.25%,注射剂应用比例为28.75%;不合理大金额处方184张,不合理率为15.33%,其中,用药不合理处方(包括用药不适宜或超常处方)73张,用药不合理率为6.08%;不规范处方111张,不规范处方率为9.25%。结论该院大金额处方存在一定不合理用药现象,有待进一步改善。Objective To improve the rational use of drugs in hospital by investigating the expensive outpatient prescriptions.Methods A total of 1200 expensive outpatient prescriptions （more than 300 yuan） in 2010 were randomly selected from a hospital with the retrospective study method,and the prescription contents,including the outpatient basic information,prescription diagnosis＇ prescriptive drug and rationality were investigated based on these regulations of prescription evaluation system.Results The number of man patients was basically equal to the woman patients in the investigation,and the number of the patients aged ＞ 50 was the most of all,accounted for 67.75％.The average cost of expensive prescription was 424.95 yuan,and 3.55 kinds of drugs were used in each prescription averagely.The rates of antibacterial prescriptions and injection prescriptions were 26.25％ and 28.75％,respectively.There were 184 irrational expensive prescriptions,existing irrational drug use and other problems,which accounted for 15.33％.The number of irrational drug use prescription was 73 pieces,accounted for 6.08％,and other problems were discovered in 111 expensive prescriptions,accounted for 9.25％.Conclusion Irrational drug use of expensive prescriptions is present in the hospital,and these shortcomings need to be further improved.
