规范化疼痛干预对吻合器痔上黏膜环切术术后疼痛的临床效果研究

摘　　要：目的探讨规范化疼痛护理干预在吻合器痔上黏膜环切术(PPH)后患者护理中的重要意义和临床应用效果。方法将2012年6月～2013年6月深圳市宝安区观澜人民医院神经外科收治的300例行PPH术的痔疮患者均分为两组,每组150例。观察组进行综合疼痛护理干预,对照组不进行干预,分别在术后3 d对患者术后疼痛进行数字分级法(NRS)评分、睡眠满意度比较,统计观察组患者疼痛缓解程度,并根据休斯顿疼痛情况调查表对患者满意度进行评价。结果观察组术后3 d疼痛NRS评分均低于对照组,睡眠满意度值均高于对照组,均存在显著差异(均P<0.05)。观察组患者的有效疼痛缓解率为92.7%,对照组患者的有效疼痛缓解率为81.3%,差异有统计学意义(χ2=5.97,P=0.019);观察组疼痛控制总体满意度为(8.5±1.7)分,对照组为(7.1±1.8)分,差异有统计学意义(P<0.05)。结论规范化疼痛干预能有效缓解PPH患者术后疼痛,提高睡眠满意度,以及患者满意度,应在临床护理中大力提倡。Objective To observe the significance and clinical therapeutic efficacy of standardized pain intervention for postoperative procedure for prolapse and hemorrhoids pain management.Methods 300 cases in the Neurosurgery Department of Guanlan People＇s Hospital of Baoan District in Shenzhen from June 2012 to June 2013 of treating hemorrhoids by procedure for prolapse and hemorrhoids were randomized into two groups averagely （n =150）.The observation group was taken integrated pain intervention while the control group was not.The differences of pain nursing record sheets （NRS） scores,postoperative sleep satisfaction in three days after surgery were compared,and the pain relif degrees of the observation group in three days after surgery and Houston pain outcome instrument （HPOI） before discharge were also recorded.Results NRS scores of the observation group were lower than the control group,while the sleep satisfaction was higher.There were significant deviations of average postoperative pain and sleep satisfaction betweem the two groups （all P ＜ 0.05）.The effective pain relif of the observation group and the control group were 92.7％ and 81.3％ respectively,the difference was significant （x2=5.97,P =0.019）,and the integrated satisfaction of the standardized pain intervention were （8.5±1.7） points and （7.1±1.8） points.There were significant differences between the two groups （all P ＜ 0.05）.Conclusion Standardized pain intervention has a vital role to play in postoperative proce dure for prolapse and hemorrhoids pain management,in raising the sleep satisfaction and integrated satisfaction,and consequently it＇s worthy to spread in clinical application continually.
