摘  要:目的探讨异丙酚全凭静脉麻醉对急性颅脑损伤手术患者血清S100B蛋白和白介素(IL)-1水平的影响。方法选择68例急诊手术治疗的颅脑外伤患者随机分为异丙酚组与对照组,分别予异丙酚和咪唑安定全凭静脉麻醉。观察并比较两组患者术前、术中1 h和术毕时血清S100B蛋白和IL-1水平的变化。结果两组患者术中1 h和术毕时MAP和HR均较术前明显上升(P<0.05或P<0.01),且异丙酚组的上升值较对照组低(P<0.05);同时两组患者术中1 h和术毕时血清S100B蛋白和IL-1水平均较术前明显上升(P<0.05或P<0.01),且异丙酚组的上升值较对照组低(P<0.05)。结论异丙酚全凭静脉麻醉能明显降低急性颅脑手术患者S100B蛋白和IL-1水平,减少脑细胞损伤,具有良好脑细胞保护作用。Objective To discuss influence of total intravenous anesthesia (TIVA) with propofol on serum S100B protein and interleukin-1 (IL-1) levels of patients treated with acute craniocerebral injury operation. Methods Sixty- eight cases of patients with acute craniocerebral injury operation were divided into propofol group and control group at random, and treated with TIVA with propofol and midazolam respectively. The changes of mean arterial pressure (MAP), heart rate (HR), serum S100B protein and IL-1 levels of patients in two groups before the operation, within one hour upon the beginning of operation and after the operation were observed and compared. Results The serum MAP and HR of patients in two groups within one hour upon the beginning of operation and after the operation were rose obviously than before (P 〈 0.05 or P 〈 0.01), and the rising rate in propofol group was much lower than that in control group (P 〈 0.05). Meanwhile the serum SIOOB protein and IL-1 levels of patients in two groups within one hour upon the beginning of operation and after the operation were rose obviously than before (P 〈 0.05 or P 〈 0.0i), and the rising rate in propofol group was much lower than that in control group (P 〈 0.05). Conclusion The TIVA with propofol can obviously decline the S100B protein and IL-1 levels of patients treated with acute craniocerebral injury operation, reduce the damages of brain ceils, and have the protection effect on brain ceils as well.
