黄芪桂枝五物汤治疗2型糖尿病周围神经病变40例效果观察

摘　　要：目的探讨黄芪桂枝五物汤治疗2型糖尿病周围神经病变的效果。方法选择2011年2月—2013年2月收治的2型糖尿病周围神经病变患者80例,随机分为对照组和观察组各40例。两组均给予饮食和运动指导,口服降糖药或皮下注射胰岛素治疗。对照组在此基础上给予甲钴胺0.5 mg/次口服,3次/d。观察组在对照组基础上加用黄芪桂枝五物汤,水煎服,日1剂。两组均治疗6个月,评价两组治疗前后正中神经和腓神经传导速度、治疗后症状消失时间及疗效。计量资料采用t检验,计数资料采用χ2检验,P<0.05为差异有统计学意义。结果观察组治疗后正中神经和腓总神经传导速度[(64.00±5.63)、(44.51±4.56)m/s]快于对照组[(57.57±4.42)、(41.55±5.17)m/s],比较差异均有统计学意义(均P<0.05)。两组症状消失时间对照组(5.2±0.5)个月,观察组(3.6±0.2)个月,比较差异有统计学意义(P<0.05)。总有效率对照组55.0%,观察组80.0%,两组比较差异有统计学意义(P<0.05)。结论黄芪桂枝五物汤治疗2型糖尿病周围神经病变可改善神经传导速度、缓解临床症状,并显著提高疗效。Objective To discuss the clinical efficacy of Huangqi Guizhi Wuwu Decoction for peripheral neuropathy in type 2 diabetes mellitus.Methods Eighty type 2 diabetic cases with peripheral neuropathy treated from February 2011 to February 2013 were randomly divided into observation group and control group,40 cases for each group.Both groups were given diet and exercises guidance,oral hypoglycemics or insulin for hypodermal injection.In addition,control group was given methylcobalamin 0.5 mg rid,and observation group was given Huangqi Guizhi Wuwu Decoction once daily.After a 6-month treatment,both groups were evaluated for median and peroneal nerve conduction velocity,time for symptom disappearanee and clinical efficacy.T-test was used for measurement data and X2 test for count data,the result of P〈0.05 was considered statistically significant.Results The conduction velocity of median and peroneal nerve were（64.00 ± 5.63）and（d4.51± 4.56）m/s respectively,which was faster than that in control group[（57.57 ±4.42）,（41.55 ±5.17）m/s],showed significant difference（P〈0.05）.The time for symptom disappearance was （5.2 ±0.5）months in control group and（3.6 ± 0.2）months in observation group,which showed significant differenee（P〈0.05）.The total effective rate was 55.0% in control group and 80.0% in observation group,which showed significant difference（P〈0.05）.Conclu- sion For peripheral neuropathy in type 2 diabetes mellitus,Huangqi Guizhi Wuwu Decoction can improve nerve conduction ve- locity,relieve symptoms,and significantly improve clinical efficacy.
