杜冷丁配伍利凡诺和米非司酮应用于中期妊娠引产效果研究

摘　　要：目的探讨杜冷丁配伍利凡诺和米非司酮在中期妊娠引产中的应用效果。方法选择2011年1月-2012年12月280例妊娠13-28周健康妇女，随机分为观察组和对照组各140例。两组均羊膜腔注射利凡诺100mg，当日空腹口服米非司酮50mg／次，3次，d。观察组发动规律宫缩宫121开大1am左右时，一次性肌内注射杜冷丁75mg。观察两组引产时间、出血量、刮宫情况、引产成功率及不良反应。计数资料采用矿检验，P〈0．05为差异有统计学意义。结果观察组引产时间〈3h34．3％，3-6h38．6％，6-12h21．4％，12～24h4．3％，≥24h1．4％；对照组引产时间〈3h20．7％。3-6h10．0％，6。12h40．O％．12-24h25．0％，≥24h4．3％，两组比较差异有统计学意义（P〈0．05）。刮宫率观察组14．29％，对照组24．29％，两组比较差异有统计学意义（P〈0．05）。结论杜冷丁配伍利凡诺和米非司酮用于中期妊娠引产安全有效，只要加强管理，合理使用，是非常理想的配合引产药物，值得临床推广应用。Objective To investigate the feasibility of pethidine combined with rivanol and mifepristone in treating induced abor- tion in mid-term pregnancy.Methods Two hundred and eighty healthy cases of 13-28 week＇s gestation were selected from Jan- uary 2011 to December 2012 and randomly divided into observation group and control group,one hundred and forty cases for each.In both groups,amniotic injection of rivanol 100 mg was given,as well as fasting oral mifepristone 50 mg,tid on the same day. When regular contractions started and the cervix opened to l cm,in observation group pethidine 75 mg was injected intramuscu- laxly at once.Both groups were observed for the length of induced labor,bleeding,and curettage rate,success rate of induced labor and adverse reacfions x^2 test was used for count data,the result of P〈0.05 indicated the difference was statistically significant.Re- sults Observation group： induced labor time was 〈3 h 34.3%,3-6 h 38.6%,6-12 h 21.4%,12-24 h 4.3% and 〉124 h 1.4%. Control group：induced labor time was 〈3 h 20.7%,3-6 h 10.0%,6-12 h 40.0%,12-24 h 25.0%, ≥24 h 4.3%.The difference was statistically significant between the two groups （P〈0.05）.Curettage rate was 14.29% in observation group and 24.29% in the con- trois,the difference was statistically significant between the two groups（P〈0.05）.Conclusion Demerol is safe and effective for in- duced labor in mid-term pregnancy in combination with rivanol and mifepristone,which is an ideal drug for induced labor and worthy of clinical popularization under strict management.
