糖化低密度脂蛋白与糖尿病合并颈动脉粥样硬化的关系

摘　　要：目的　观察糖化低密度脂蛋白 (Gly -LDL)与糖尿病合并颈动脉粥样硬化 (CAS)的关系。方法通过询问病史、血糖测定和颈动脉复式超声检查确诊 2型糖尿病 (2型DM)合并CAS病人 32例。对照组 30例。采用氨基苯硼酸琼脂糖凝胶亲合层析法测定血浆Gly -LDL含量。结果　 2型DM合并CAS组Gly -LDL水平为(8 89± 2 5 0 ) %。对照组Gly -LDL水平为 (4 0 5± 1 0 8) %。 2型DM合并CAS组Gly -LDL水平显著高于对照组(P <0 0 0 1) ,且氧化修饰低密度脂蛋白 (Ox -LDL)与Gly -LDL含量呈正相关 (P <0 0 0 1) ,颈动脉狭窄率与Gly-LDL含量呈正相关 (P <0 0 0 1)。结论　Gly -LDL水平与 2型DM病人动脉粥样硬化 (AS)形成有密切关系。糖化促进氧化是 2型DM病人发生AS的重要发病机制之一。Gly -LDL水平可反映CAS的程度 。Objective To observe the relation between Glycosylated low density lipoprotein(Gly-LDL) and diabetes with carotid atherosclerosis(CAS).Methods Thirty-two type 2diabetes mellitus (DM2) with CAS patients were diagnosed according to case history and blood-glucose measurement and carotid ultrasonic examination.Thirty control subjects without the clinical signs of diabetes and AS were chosen by means of physical examinations and carotid ultrasonic examination.Gly-LDL was measured by aminophenylborate gel affinity chromatography.Results The level of plasma Gly-LDL was (8 89±2 50)% in DM2 with CAS patients The level of plasma Gly-LDL was (4 05±1 08)% in control subjects.The level of Gly-LDL was significantly high in DM2 with CAS group compared with control subjects (P<0 001).There was a significantly positive correlation between the level of Ox-LDL and the level of Gly-LDL in DM2 with carotid AS group (P<0 001).There was a significantly positive correlation between the carotid stenotic rate and the level of Gly-LDL in DM2 with carotid AS group (P<0 001).Conclusion There is a close relation between the level of Gly-LDL and AS of DM2 patients.Oxidation of LDL promoted by glycosylation is one of important pathogenesis of AS formation in DM2 patients.The level of plasma Gly-LDL may represent the degree of CAS.
