慢性肺心病心肺功能改变对肺动脉压的影响

摘　　要：目的 :观察研究 48例慢性肺心病者心肺功能变化时与肺动脉高压 (PAH)的相互关系。方法 :采用无创法测定患者心肺功能和动脉血气分析。结果 :慢性肺心病者伴PAH时肺的通气功能和右心功能如 %VC、FEV /VC、%V50及 %V2 5明显下降 ,且RV/TLC明显增加及EF、CO下降 ,而PVR、RVEP明显增加 ,并与PAH升高程度呈密切相关 (P <0 0 1)。结论 :慢性肺心病者缺氧、二氧化碳潴留可引起肺血管阻力增加 ,肺动脉压升高。提示肺功能与肺循环变化之间存在着密切相关。Objective:Investigation was correlated between right heart function and pulmonary function parameter in 48 cases of patients with chronic cor pulmonale with PAH. Methods: Right heart function and pulmonary function parameter and blood gas analysis were measured in the patients. Results: MPAP in the patients with chronic cor pulmonale had significant correlation with condition of increase of PAH,%VC, FEV 1/VC,%V 50 , %V 25 of the pulmonary function paraemerter were decreased and RV/TLC of the pulmonary function parameter was increased and EF, CO of the right heart function were decreased and PVR, RVEP of the right heart function in the patients were increased. Conclusion: The right heart function had significant correlation with the pulmonary function parameter in the patients with chronic cor pulmonale with PAH.It can be explained that the decrease or disturbance of the pulmonary function parameter has effected on pulmonary circulation.
