原发性肾病综合征患儿血小板活化状态及复方丹参滴丸疗效观察

摘　　要：目的　观察原发性肾病综合征 (PNS)患儿血小板活化状态的变化并探讨复方丹参滴丸 (CSDP)的临床疗效。方法　将 42例PNS患儿随机分为CSDP组 2 2例和对照治疗组 2 0例 ,选择 2 0例门诊健康体检小儿为正常组。CSDP组予CSDP ,学龄前儿童每次 3粒 ,学龄儿童每次 5粒 ,均每日 3次 ,两组均同时服用强的松 2mg/(kg·d) ,治疗 8周 ,治疗前后采用ELISA法检测血浆α 颗粒膜蛋白 (GMP 140 )水平 ,并观察血小板计数变化 ,及蛋白尿转阴、水肿消退时间。结果　治疗前 ,CSDP组、对照组血浆GMP 140水平明显高于正常组 (P <0 0 1) ;治疗后 ,CSDP组蛋白尿转阴、水肿消退时间较对照组明显缩短 (P<0 0 5 ,P<0 0 1) ,同时GMP - 140较对照组显著下降 (P <0 0 1) ,与正常组无差别 (P >0 0 5 ) ,对照组部分患儿GMP - 140水平仍高于正常组 (P <0 0 5 )。血小板计数差异则无显著性。结论　PNS患儿存在血小板的活化 ,CSDP可抑制PNS患儿血小板活化功能的异常而改善症状。Objective To study the changes of platelet granule membrane protein-140(GMP-140)in children with primary nephrotic syndrome(PNS)and to evaluate the effects of Composite Salviae Dropping Pill(CSDP)in treating the disease. Methods 42 PNS patients were randomly divided into two groups-CSDP treated group (n=22)and control treated group(n=20). CSDP was given to CSDP treated group orally for 8 weeks.Prednisone 2mg/(kg·d) and other conservative agents were given to both groups. 19 health children were served as normal control group. The levels of GMP-140 was measured by ELISA. Results The results showed that the plasma level of GMP-140 in PNS was significantly higher than that in control groups(P<0 01). The level of GMP-140 corresponded to the severity of PNS. Following the treatment,CMP-140 level decreased significantly in CSDP group compared with that in control treated group(P<0.01).There was no significant difference in platelet count among the groups.Conclusion There finding indicate that there were elevated platelet activation in PNS,and CSDP may inhibit the activation of platelet,therefore improve the symptoms and cut off the development of PNS.
