干体重达标对左心室心肌质量指数的影响

摘  要:目的探讨干体重是否达标对左室心肌质量指数(LVMI)的影响及其与血压变化之间的关系。方法 45例维持性血液透析患者随机分为超滤组(30例)和对照组(15例),于初始时及4周、8周时采集血压及超声心动图指标进行对比分析。结果初始时,对照组和超滤组的LVMI分别为136.3 g/m2和138.7 g/m2,2组差异有统计学意义(P>0.05)。4周及8周时分别上升了3.5 g/m2和0.4 g/m2(P>0.05)。而在超滤组,4周及8周时分别下降了7.5 g/m2和6.3 g/m2。比对照组4周时多下降11.0 g/m2(P<0.05),8周时多下降6.7 g/m2(P>0.05)。而初始时LVMI较高的患者中超滤组的血压下降更明显(P<0.05)。结论干体重达标可降低善血透患者的LVMI,从而改善其预后。Objective To investigate the effect of probing dry-weight on left ventricular mass index (LV- MI) and to explore its relationship with blood pressure changes. Methods Forty-five patients who underwent maintenance hemodialysis were randomly divided into ultrafihration group ( n = 30) and control group ( n = 15 ), the blood pressure in all the subjects was detected and echocardiogram indexes were analyzed at beginning,4 weeks and 8 weeks. Results At beginning, LVMI in control group and uhrafihration group was 136. 3g/m2 and 138.7g/m2 ,respectively,there was a significant difference between two groups ( P 〈0.05 ) ,which at 4 weeks and 8 weeks in control group was increased by 3.5g/m2 and 0.4g/m2, respectively ( P 〈 0.05 ). However LVMI at 4 weeks and 8 weeks in uhrafihration group was decreased by 7.5g/m2 and 6.3g/m2, respectively, the decrease degree was 11.0g/m2 more than that in control group at 4 weeks ( P 〈 0.05 ) and 6.7 g/m2 at 8 weeks ( P 〈 O. 05). Furthermore at beginning the blood pressure of patients with higher LVMI was more obviously decreased (P 〈 O. 05). Conclusion The probing dry-weight up to the standard can decrease LVMI of patients undergoing hemodialysis, as a result, which can improve patient's prognosis.
