可控弯度鞘管在心房颤动环肺静脉电隔离术中的应用

摘　　要：目的:总结22例心房颤动环肺静脉电学隔离术中使用可调弯鞘的护理体会。方法:选择适当型号的可调弯鞘,术中做好鞘管的护理,重点是整个手术过程的肝素盐水鞘内灌注,以减少血栓栓塞等并发症的发生。结果:22例患者均完成4根肺静脉电学隔离术,无1例患者术中或术后出现血栓栓塞、心填压塞。3例患者出现迷走神经反应,经对症处理症状缓解。1例发生穿刺部位血肿,未予特殊处理,自行吸收。结论:术中对可调弯鞘进行细致周到的护理非常重要,不仅能提高手术效率,还能减少血栓栓塞等并发症发生。Objective：To summarize nursing experience of Agilis sheath which used for pulmonary veins isolation in 22 patients with atrial fibrillation.Methods：Proper type Agilis sheath was used for pulmonary isolation in 22 patients with atrial fibrillation.Heparin salt was used for Agilis sheath irrigation to prevent complications such as thrombosis or during the whole procedure.Results：Four pulmonary veins were isolated successfully in all 22 patients.Thrombosis and cardiac tamponade did not occur in any patient.However,vagus nerve response happened in 3 patients.With immediately treatment,they all recover soon.Puncture site hematoma occurred in one patient.Conclusion：It is very important to pay effective nursing for Agilis sheath during the procedure,which not only improves ablation success rate,but reduces complication occurrence.
