摘  要:目的探讨应用携酸性成纤维细胞生长因子(aFGF)的脂质微泡结合超声靶向击破技术对糖尿病心肌病(DCM)大鼠左室收缩功能的保护作用。方法64只健康雄性SD大鼠中52只通过腹腔注射链脲佐菌素建立DCM模型,其余12只作为正常对照组。DCM大鼠随机平均分为4组进行相应干预:DCM模型组、单纯aFGF用药组、Sono Vue-aFGF组及Sono Vue-aFGF+超声辐照组。干预前及干预后4周所有大鼠均行M型超声心动图及速度向量成像(VVI)检查,测定左室射血分数(LVEF)、左室短轴缩短率(LVFS)、收缩期乳头肌水平左室壁平均圆周应变(Sc)及径向应变(Sr)。处死大鼠后取心肌组织,行CD31免疫组织化学染色检测心肌微血管密度(MVD)。结果干预前,各实验组LVEF、LVFS、Sc及sr均明显低于正常对照组(P〈0.01)。干预后4周,Sono Vue-aFGF+超声辐照组大鼠Sc、Sr及MVD均较DCM模型组明显增高(P〈0.01),而单纯aFGF用药组和Sono Vue-aFGF组上述指标较DCM模型组略有增高(P〉0.05)。干预后4周,Sono Vue—aFGF+超声辐照组大鼠Sc、Sr较干预前明显增高(P〈0.05~0.01),DCM模型组上述指标较干预前明显减低(P〈0.05~0.01),单纯aFGF用药组和Sono Vue-aFGF组上述指标较干预前略有增高(P〉0.05)。结论超声联合脂质微泡靶向递送aFGF能有效改善DCM大鼠的左心室收缩功能。Objective To investigate the protective effects of left ventricular systolic function after delivery of acidic fibroblast growth factor (aFGF) in diabetic cardiomyopathy(DCM) in rats by using ultrasound-targeted microbubble destruction (UTMD) with velocity vector imaging(VVI). Methods The sixty-four male SD rats comprised fifty-two DCM rats, which were induced by streptozotocin through intraperitoneal injecting,and twelve normal rats as normal control group in the study. The DCM rats were randomly divided into four groups and processed as follows:DCM model group, aFGF only group, SonoVue- aFGF group, SonoVue-aFGF + UTMD group. Before and four weeks after intervention,all rats underwent M-mode echocardiography and VVI exams. Left ventricular ejection fraction (LVEF) ,left ventricular short axis shorten rate (LVFS) were measured using M-mode echocardiography. The mean peak circumferential strain (Sc) and radial strain (Sr) of six wall at the papillary muscle level were measured in left ventricular short axis view by VVI. At last, myocardial tissue of all rats were stained with CD31 immunohistochemistry to quantify myocardial microvascular density(MVD). Results Before intervention, LVFS, LVEF, Sc and Sr in each experimental group were significantly lower than in the normal control group (P 〈0.01). Four weeks after intervention, Se, Sr and MVD in the SonoVue-aFGF + UTMD group were significantly increased than in the DCM model group ( P 〈0.01) ,and the above indexes slightly increased in both aFGF only group and SonoVue-aFGF group than those of DCM model group ( P 〉0.05). Compared with before intervention,Sc and Sr in the SonoVue-aFGF + UTMD group were significantly higher and these in the DCM model group were significantly lower ( P 〈0.05 - 0.01) ,in the aFGF only group and SonoVue-aFGF group were slightly increased( P 〉0.05).
