血浆vWF:Ag、AT:A和t-PA水平与小儿过敏性紫癜疾病凝血纤溶活性的关系

摘　　要：目的探讨血浆血管性血友病因子抗原(vWF:Ag)、抗凝血酶活性(AT:A)和组织型纤溶酶原激活剂(t-PA)水平与小儿过敏性紫癜疾病(HSP)凝血纤溶活性的关系。方法选择HSP患者94例,按照D-二聚体(D-D)是否阳性分为D-D阳性组(49例)和D-D阴性组(45例);另选同期的健康就医者50例为健康对照组。利用全自动血凝仪,测定各组血浆vWF:Ag、AT:A、t-PA水平。结果 HSP两组与健康对照组比较,AT:A和t-PA水平降低,vWF:Ag水平显著增高,差异有统计学意义(P<0.01)。D-D阳性组vWF:Ag水平显著高于D-D阴性组,而AT:A和t-PA水平显著低于后者,差异均有统计学意义(P<0.01)。两试验组的AT:A和t-PA水平呈正相关(r=0.917,P<0.01;r=0.893,P<0.01)。结论血浆vWF:Ag水平与HSP疾病发展过程中的血管内皮细胞变化有关,AT:A和t-PA水平与HSP疾病发展过程中的凝血和纤溶活性有关,三项生物指标对HSP疾病的诊断、预防和疗效观察具有重要的意义。Objective To study the relationship between the levels of the plasma vWF：Ag, AT：A and t-PA and the procoagulant activity and the fibrinolytic activity in the children with Henoeh-Schonlein purpura. Methods The levels of the plasma vWF：Ag, t-PA and AT：A were measured in 49 cases of D-D positive group and 45 eases of D-D negative group. Fifty healty individuals were selected as the healthy control group. The changes of various markers such as the plasma vWF：Ag, t-PA and AT：A were compared and analyzed. Results Compared with those in the healthy control group, the levels of AT：A and t-PA in D-D positive group and D-D negative group decreased signifi- cantly, and the level of vWF：Ag increased significantly, with statistically significant difference （1~〈0.01）. The level of vWF：Ag was significantly higher in D-D positive group than D-D negative group, but the levels of AT：A and t-PA were significantly lower （P〈0.01）. The levels of AT：A and t-PA were positively correlated in the two groups （^--0.917, P〈0.01; r=0.893, P〈0.01）. Conelution The level of plasma VWF：Ag is related to the changes of the vascular endothelial cell, and the levels of plasma t-PA and AT：A are related to the procoagulant activity and the fibrinolytic activity in disease development process of Henoeh-Schonlein purpura.
