血浆和肽素、PAMP、NT-proBNP水平与慢性心力衰竭及其严重程度的相关性

摘　　要：目的探讨血浆和肽素、肾上腺髓质素前体N端20肽(PAMP)及N末端原脑利钠肽(NT-proBNP)水平与慢性心力衰竭及其严重程度的相关性。方法选择我院2011年1月至2013年1月收治的35例慢性心力衰竭(CHF)患者,分别测定血浆和肽素、PAMP及NT-proBNP水平,选取同期健康体检者35例作为对照,观察比较两组间的检测结果并进行统计学分析。结果观察组患者血浆和肽素、NT-proBNP水平与对照组相比明显升高(P<0.01),且心功能Ⅳ级患者与心功能Ⅱ级、Ⅲ级患者比较,明显增高,差异具有统计学意义(P<0.05)。血浆和肽素分别与LVEDD值(r=0.53)、NYHA分级(r=0.54)、NT-proBNP(r=0.42)呈显著正相关(均P<0.05);且与左室射血分数呈负相关(r=-0.67,P<0.05)。心力衰竭患者血浆PAMP水平与对照组比较差异无统计学意义(P>0.05)。结论血浆和肽素、NT-proBNP水平在慢性心力衰竭中显著升高,并与慢性心力衰竭的严重程度密切相关。Objective To analyze the correlation of plasma, peptide hormones, proadrenomedullin N-termi- nal 20 peptide （PAMP）, N-terminal probrin natriutic peptide （NT-proBNP） level with chronic heart failure and its se- verity. Methods Thirty-five chronic heart failure patients in our hospital from January 2011 to January 2013 were chosen as the research subjects （the study group）. Plasma, peptide hormones, PAMP and the NT-proBNP level were de- termined. Thirty-five healthy individuals were selected as the control group. The test results of the two groups were ob- served and statistically analyzed. Results Plasma, peptide hormones, and the NT-proBNP level were significantly higher in the study group than the control group （P〈0.01）, in patients with cardiac function grade II and l~ than those with cardiac function grade IV （P〈0.05）. The peptide hormones was positive correlated with LVEDD value （r--0.53）, NYHA classification （r=0.54） and the NT-proBNP （r=0.42, P〈0.05）, and negatively correlated with left ventricular ejection fraction （r=0.67, P〈0.67）. Conclusion Plasma, peptide hormones, NT-proBNP levels increase significantly in patients with chronic heart failure, and they are closely related to the severity of chronic heart failure.
