摘　　要：目的:探讨分析我院肿瘤药房静脉用药集中调配中心的不合理医嘱,促进临床合理用药。方法:通过查阅药品说明书及相关文献资料,对肿瘤药房静脉用药不合理医嘱的典型案例进行整理与分析。结果:在医嘱审核中最常见的问题是溶媒不适宜,药品的稳定性、溶解度、渗透压等是决定药品溶媒类别、载体量的重要因素。结论:药师应加强对医嘱的审核与分析,提高专业知识水平,为临床合理用药提供保障。Objective：To analyze the irrational medical orders of pharmacy intravenous admixture services （PIVAS） of tumor pharmacy in order to promote rational use of drugs. Methods：The typical cases of unreasonable orders from PIVAS were analyzed by studying the drug-related information and drug instructions. Results：The main problem of unreasonable medical orders was improper solvent. The type and volume of solvent were depended on the basic properties of drugs, such as stability, solubility, permeability and so on. Conclusion：Pharmacists should strictly examine medical orders via using expertise knowledge in order to ensure safe and rational use of drugs.
