伊曲康唑联合特比萘芬间歇冲击疗法治疗甲真菌病的疗效

摘　　要：目的探讨伊曲康唑联合特比萘芬间歇冲击疗法治疗甲真菌病的临床疗效。方法将188例甲真菌病患者按随机数字表法分为对照组和观察组,每组94例。对照组给予伊曲康唑胶囊200 mg,口服,2次·d-1,连续服用7 d,停药21 d为1疗程;其中指甲真菌病患者连续服用2个疗程,趾甲真菌病或者指甲合并趾甲真菌病的患者连续服用3个疗程。观察组采用伊曲康唑胶囊联合特比萘芬片间歇冲击疗法治疗,伊曲康唑200 mg+盐酸特比萘芬片250 mg,口服,1次·d-1,连续服用7 d,停药21 d为1个疗程,连续服用3个疗程。对2组的疗效、真菌学治愈率、复发率及不良反应进行比较。结果观察组近期(2011年1—6月)、远期(2011年7月至2012年7月)治疗总有效率以及真菌学治愈率略高于对照组,但2组比较差异均无统计学意义(均P>0.05)。临床和真菌学治愈后随访6个月,观察组复发率为8.1%,对照组复发率为31.1%,2组比较差异有统计学意义(P<0.05)。2组均无严重不良反应发生。结论伊曲康唑联合特比萘芬间歇冲击疗法治疗甲真菌病,具有疗效确切、安全性好和复发率低等特点,两种药物联合应用,可对甲真菌病发挥协同的治疗作用。Objective To observe the clinical effects of intermittent pulse therapy with itraconazole and terbinafine on onychomycosis. Methods A total of 188 onychomycosis patients were randomly divided into two groups, with 94 patients in each group. In control group, patients with fingernail infections and patients with toenail or/and fingernail fungal fungal infections were given 2 and 3 courses of treatment with oral itraconazole （200 mg） twice per day for consecutive 7 days,respectively. In observation group, patients were given 3 courses of intermittent pulse therapy with oral itraconazole（200 mg）and terbinafine（250 mg）once per day for consecutive 7 days. Courses were repeated every 21 days. Curative effects, mycological cure rates, recurrence rates and adverse reactions were compared between the two groups. Results The short-term （from January 2011 to June 2011 ） and long-term （from July 2011 to July 2012） effective rates,and mycological cure rates in observation group were higher than those in control group,but the differences were not significant （all P〉0.05）. The recurrence rates in observation group was significantly lower than those in control group （P〈0.05）. No serious adverse reactions were found in both groups. Conclusion Itraconazole plays a synergistic role with terbinafine in intermittent pulse therapy of onychomycosis. The combination of itraconazole and terbinafine is an effective and safe treatment and results in low recurrence rate in onychomycosis.
