摘 要:目的研究根动脉输注不同浓度左旋布比卡因对脊髓神经元特异性烯醇化酶(NSE)、S100β蛋白、Tau蛋白的影响,初步探索神经内局麻药大概的毒性最低浓度及神经毒性机制。方法 25只健康实验犬,随机分为5组,每组5只,对照组(S组:0.9%NaCl);不同浓度左旋布比卡因(LB)组(B1:0.0625%,B2:0.125%,B3:0.25%,B4:0.375%左旋布比卡因),经肋间后动脉置管至椎间孔外口附近给药,各组缓慢注入3 ml后用恒速泵经连接导管以3 ml/h的速度持续泵注3 h。取相应部位的脊髓约1 cm,10%缓冲甲醛液固定,24 h后常规石蜡包埋,制成蜡块。做尼氏染色、免疫组织化学,观察组织学变化、尼氏小体及尼氏染色阴性神经元细胞计数,观察NSE、S100β蛋白、Tau蛋白的表达并用图像分析仪测量其免疫阳性物的平均吸光度值(A)。结果与对照组比较,B1组尼氏染色阴性神经元细胞计数、NSE、S100β蛋白、Tau蛋白的表达无明显变化,差异无统计学意义(P>0.05);B2、B3、B4组尼氏染色阴性神经元细胞计数、NSE、S100β蛋白、Tau蛋白的表达增加,差异有统计学意义(P<0.01)。与B1组比较,B2、B3、B4组尼氏染色阴性神经元细胞计数、NSE、S100β蛋白、Tau蛋白的表达增加,差异有统计学意义(P<0.05)。结论犬根动脉内持续输注0.125%左旋布比卡因3 h,即可引起脊髓神经元和神经胶质细胞的损伤。神经毒性的机制可能与Tau蛋白的过度表达有关。Objective To investigate the effects of different concentrations of levobupivacaine maintenance in- jection via radieular artery on spinal cord neural non-specific enolase (NSE) , S100β and Tan protein expression in canines, and to explore the minimal neurotoxicity concentration and mechanism of toxicity of local anesthetics within the spinal cord. Methods Twenty-five healthy dogs were randomly allocated to he treated with natural saline injec- tion in control group (group S, n=5) and different concentrations of levobupivacaine in four groups (group BI: 0.0625%, n=5; group B2: 0.125%, n=5; group B3: 0.25%, n=5; group B4: 0.375%, n=5), respectively. Bolus injection was achieved by using the posterior intercostal artery catheter, followed by continuous infusion at a rate of 3 ml/h for 3 hours. This was followed by extraction of 1 cm spinal cord for fixation with buffer solution containing 10% formalde- hyde, which was embedded in solid paraffin. The Nisll staining was employed to determine the histological changes, Nis]l bodies and the neurons free of Nisll body. The immunohistochemical assay was applied to investigate the dynam- ic changes in NSE, S100[~ and Tan protein expression in spinal cord tissues, and the mean optical density (MA) of positive substances was calculated by using an imaging system. Results Compared with group S, the group B1 did not yield statistically higher number of Nisll-negative neurons nor augmented expression of NSE, S10013 and Tau pro- tein (all P〉0.05). However, groups B2, B3 and B4 elicited markedly increased number of Nisll-negative neurons nor augmented expression of NSE, S100β and Tau protein compared with group S (all P〈0.05). This held true for compar- ison of group B1 versus groups B2, B3 and B4 (all P〈0.05). Conclusion Continuous injection of 0.125% levobupi- vacaine via the radicular artery for 3 hours elicits injury of spinal cord neurons and neuroglia cells, which was likely to he associated with the over-expression of Tau protein
