摘  要:目的研究脱钙人牙基质(decalcified tooth matrix,DTM)复合胶原材料对实验性兔股骨缺损的修复作用。方法新西兰大白兔24只,随机分为2组,无菌条件下麻醉动物,于股骨远端钻孔制备直径约6mm、深约5mm的骨缺损模型;实验组于缺损区植入脱钙人牙基质复合胶原材料,对照组不进行任何植入处理。术后1、4、8、12周每组分别处死3只动物,进行大体观察、同步辐射X射线观察及组织学观察,并对成骨量进行分析。结果从大体观察结果来看,术后12周,实验组新生骨长入骨缺损处,植入材料与周围骨组织间界面基本消失,而对照组骨缺损处明显凹陷。同步辐射X射线观察显示,实验组术后4周材料开始降解吸收;术后8周骨缺损区密度已基本接近宿主骨;术后12周骨缺损区密度与正常骨骼无明显差异。组织学观察结果显示,实验组术后4周新生骨组织开始生长;术后8周后可见大量成骨细胞和破骨细胞,新生骨向材料内生长;术后12周缺损部位已经长出成熟的骨组织,骨小梁融合并且排列致密。成骨量分析结果显示,在术后4、8、12周,实验组新生骨含量都明显高于对照组,说明该复合材料具有良好的成骨性能,对兔股骨缺损有明显修复作用。结论脱钙人牙基质复合胶原材料是较理想的支架材料,具有良好的诱导成骨作用,可作为一种新型复合材料修复骨缺损,具有良好的临床应用前景。Objective To investigate the feasibility and validity of decalcified tooth matrix(DTM)composite collagen materials as a scaffold for femur defect repair in rabbits.Methods DTM composite collagen materials were prepared by vacuum freeze drying.After induction of anesthesia,a hole,6mm in diameter and 5mm in depth,was made in the distal femur under sterile condition to establish the model of femur defects.Twenty-four New Zealand white rabbits were randomly divided into two groups:experimental group,in which the femur defects were filled with DTM composite collagen materials,and control group,in which no implants were given.Three rabbits were sacrificed at 1,4,8and 12week(s),respectively,after the operation in each group.The repair of the femur defects was observed in each group by gross pathological examination,synchroton-radiation X-ray tomographic microscopy,HE staining and Masson trichrome staining.The newly formed bone was quantitatively determined by statistically analyzing the histological sections.Results The gross pathological examination showed that the newly formed bone was indistinguishable from the original bone 12weeks after implantation.Synchroton-radiation X-ray tomographic microscopy showed that the defect area was completely filled with newly formed bone 12weeks after implantation.However, there were significant pits in the corresponding area in the control group.Histological examination revealed that the implant materials were encapsulated by bone tissues,the interface between the materials and the host bone was clearly visible,and the materials started to degrade from the edge of the implant after 4weeks.At 8weeks,some new bone tissues formed and grew into the pores of the materials,and the interface was in direct contact with the surface of the implant.The resorption of the materials continued and paralleled the new bone formation in many areas of the implant at 12weeks.The boundary between the materials and the host bone was unclear due to the formation of mature bone tissue,which bond
