医院2002-2011年住院儿童布氏菌病分析

摘　　要：目的了解儿童布氏菌病临床特征，提高医生对儿童布氏菌病认识，做到合理及时治疗。方法对太原市第三人民医院2002年1月1日至2011年12月31日确诊住院的50例儿童布氏菌病病历资料进行回顾性分析。结果2002-2011年10年，历年布氏菌病住院总数分别为18、11、16、11、24、22、37、39、49、44例，其中儿童患者分别为1例（5．6％）、0例、2例（12．5％）、0例、6例（25．0％）、3例（13．6％）、7例（18．9％）、14例（26．5％）、13例（26．5％）、4例（9．1％），2002-2010年，儿童布氏菌病占比逐年增加，2011年有所下降。患儿常见症状为发热（100．0％，0）、多汗（52．0％，26例）、乏力（56．0％，28例）、关节疼痛（44．O％，22例）多见，肝功能受损36例（72．0％）。除1例合并严重肝损同时伴脑膜脑炎者住院1d病情无明显好转转至北京继续治疗外，其余病例均治愈或病情好转出院继续治疗。结论2009-2010年太原市第三人民医院儿童布氏菌病住院构成呈逐年升高趋势，流行特点与成人相似，临床表现复杂多样，除发热、多汗、乏力、关节受累等常见表现外，肝脏受损更为多见，应引起临床医生高度重视。Objective To understand the clinical features of brucellosis in children. Methods The medical records of hospitalized brucellosis in children during the past 10 years were retrospectively analyzed. Results The total incidence of brinell disease in hospital in 2002, 2003, 2004, 2005, 2006, 2005, 2006, 2009, 2010, 2009 was 11, 11, 16, 18, 24, 22, 37, 39, 49, 44 cases, respectively; the number of children in each year was 1 case （5.6/18）, 0 case, 2 cases （12.5%）, 0 case, 6 cases （25.0%）, 3 cases （13.6%）, 7 cases （ 18.9% ）, 14 cases （26.5%）, 13 cases （26.5%） and 4 cases （9.1%）. The incidence of brinell disease in children in 2002-2010 increased year by year. Fever （ 100. 0%, 50/50 ）, sweat （ 52.0%, 26/50 ）, fatigue （56.0%, 28/50）, joint pain （44.0%, 22/50） and liver function damage in 36 cases （72%） were observed. Conclusions The instruction of hospitalized brucellosis of children showes an increasing trend. This character is similar to the adult＇s. The clinical performance showes its complex and diverse in addition to the common manifestation of fever, sweating, fatigue and joint involvement. The clinicians should pay great attention to the children brucellosis.
