摘  要:目的探讨Snare第二代抓捕器(Snare-Ⅱ)在室间隔缺损介入治疗中的安全性和有效性。方法将接受单纯膜周型室间隔缺损介入封堵术的73例患者随机分为两组:对照组34例,使用Snare-Ⅰ抓捕器;试验组39例,使用Snare-Ⅱ抓捕器,对比分析两者终点指标、效果指标和安全指标。结果两组患者基线资料的差异均无统计学意义(P均>0.05)。所有入选者均成功建立轨道,无并发症发生;试验组导丝抓捕时间为(3.98±1.91)s,X线辐照时间为(15.43±8.80)s,抓捕次数为(1.08±0.41)次,放射量为(6.78±3.76)μSv,而对照组分别为(24.89±3.60)s、(62.78±18.24)s、(4.79±0.84)次、(35.47±8.07)μSv,二者差异均有统计学意义(P均<0.01);试验组抓捕器撤回时间为(4.55±0.69)s,略长于对照组[(4.20±0.74)s,P<0.05)];两组间抓捕器送达肺动脉时间及总手术时间方面的差异均无统计学意义(P均>0.05)。结论介入治疗室间隔缺损时,采用Snare-Ⅱ抓捕器,实用性和安全性更高。Objective To assess the efficacy and safety of Snare-Ⅱ in interventional treatment of ventricular septal defect (VSD). Methods Seventy-three patients with perimembranous VSD who accepted transcatheter closure were enrolled and divided into two groups, The control group used Snare-I (n= 34), and then experimental group used Snare-Ⅱ(n= 39) for transcatheter closure of VSD. The final effect, efficacy and safety were compared between thee two groups. Results The difference of baseline data between the two groups was no significant. All selected patients were established tracks suc- cessfully, with no complication occurred. The guidewire arrest time of experimental group was (3.98± 1.91)s, and shorter than the control group ([24.89 ± 3.60Is, P〈0.01]. The X-ray irradiation trme of experimenl:al group was (15.43 ± 8.80)s, the number of arrest was 1.08±0. 41, the amount of radiation was (6.78±3.76)pSv, and that in the control group was (62. 78±18.24)s, 4.79±0.84, (35.47± 07)9Sv, respectively, all of them had signiicant difference (all P〈 0.01). The time of the arrest withdrawn in experimental group was (4.55 ± 0.69)s, and slightly longer than the control group ([4.20±0. 74]s, P〈0. 05). The time of the arrest served to pulmonary artery, operation time between the two groups had no statistical difference (all P〈0. 05). Conclusion Snare-Ⅱ is practicable and safe in interventional treatment of VSD.
