影响5年内慢性阻塞性肺疾病复发的多因素分析

摘　　要：目的探讨影响5年内慢性阻塞性肺疾病（chronic obstructive pulmonary disease,COPD）复发的相关因素。方法以218例COPD患者为研究对象,观察组患者为5年内未每年复发1次,且复发次数〈5次者;对照组患者为5年内每年至少复发1次者。对患者一般情况进行单因素分析及二元Logistic分析。结果单因素分析结果表明,COPD复发与患者的病程、吸烟、饮酒、长期氧疗、是否坚持药物治疗、是否合并SAS及SDS有关;二元Logistic分析结果表明,吸烟OR=5.018,95%CI：3.173~12.610,饮酒OR=4.162,95%CI：1.540~6.189,COPD病程OR=0.435,95%CI：0.183~1.033,坚持药物治疗OR=0.322,95%CI：0.136~0.759,抑郁OR=0.226,95%CI：0.098~0.525,焦虑OR=0.625,95%CI：0.267~1.461。结论吸烟、饮酒、COPD病程长、合并SDS是COPD复发的危险因素,而患者长期坚持氧疗、坚持药物治疗是COPD复发的保护性因素。Objective To investigate the relevant factors affecting the recurrence chronic obstructive pulmonary disease（ COPD） within five years. Methods 218 cases of COPD in our hospital were chosen as the research objects,the observation group had no yearly recurrence within 5 years and relapse frequency ＆lt; 5,the control group relapsed at least once a year in five years,and single factor analysis and binary logistic analysis of COPD were carried out. Results Single factor analysis showed that smoking,drinking, long-term oxygen therapy,drug treatment status,and combination with SAS and SDS were affecting factors of recurrence and binary lo- gistic analysis showed that affecting factors were smoking OR = 5. 018,95% CI（ 3. 173- 12. 610）,drinking OR = 4. 162,95% CI（ 1. 540-6. 189）,COPD disease course OR =0. 435,95% CI（ 0. 183-1.033）,adhere to drug therapy OR =0.322,95% CI（ 0. 136- 0. 759）,depression OR = 0. 226,95% CI（ 0. 098- 0. 525）,anxiety OR = 0. 625,95% CI（ 0. 267- 1. 461）. Conclusion Smok- ing,drinking,COPD duration,SDS are high risk factors for recurrence of COPD,long-term oxygen therapy and consistant drug therapy were protective factors for recurrence in COPD patients.
