抗环瓜氨酸肽抗体和抗突变型瓜氨酸波形蛋白抗体在早期类风湿关节炎患者血清中的表达及对其诊断价值

摘　　要：目的:探讨抗环瓜氨酸肽(CCP)抗体和抗突变型瓜氨酸波形蛋白(MCV)抗体在早期类风湿关节炎(RA)患者体内水平和对其诊断价值。方法:采用酶联免疫吸附(ELISA)法,检测了87例早期RA患者(早期RA组)、40例中、晚期RA患者(中晚期RA组)、60例其他风湿病患者(对照组)和40例健康体检者(正常组)血清抗CCP抗体和抗MCV抗体的水平。结果:抗CCP抗体和抗MCV抗体,在早期RA组分别是(89.1±32.6)RU/ml和(92.5±36.4)U/ml,在中晚期RA组分别是(68.5±21.2)RU/ml和(70.3±25.0)U/ml,在对照组分别是(35.0±15.6)RU/ml和(30.2±12.3)U/ml,在正常组分别是(25.6±9.2)RU/ml和(18.5±8.4)U/ml。在早期RA组和在中晚期RA组,抗CCP抗体敏感度分别是67.8%和60.0%,特异性分别是89.6%和91.5%;抗MCV抗体的敏感度分别是78.1%和72.5%,特异性分别是94.2%和93.1%。结论:抗CCP抗体和抗MCV抗体对诊断早期RA具有良好的敏感性和特异性,两者可作为诊断早期RA的标记物,是诊断早期RA疾病的两个新指标。Objective：To investigate the diagnostic significance of anti-cyclic citrullinated peptide （CCP） antibodies and anti- mutant citrullinated vimentin （MCV） antibody in the diagnosis of the early stage of rheumatoid arthritis （RA）. Methods： Using en- zyme-linked immunosorbent assay （ELISA） to detect the serum level of anti-CCP antibody and anti-MCV antibodies of 87 patients with RA （early RA group）, 40 patients in the middle and late stage of RA （middle and late stage RA group） ,60 patients with other rheu- matic diseases （control group） and 40 healthy subjects （normal group）. Results： anti-CCP and anti-MCV antibodies in RA group were （89.1 ± 32.6）RU/ml and （92.5± 36.4）U/ml, the middle and late stage RA group were （68.5 ± 21.2）RU/ml and （70.3 ± 25.0） U/ml; in the control group were respectively （35.0 ± 15.6）RU/ml and （30.2± 12.3）U/ml, and the normal group were （25.6 ±9.2）RU/ml and （18.5 ± 8.4） U/m1. In the RA group, the sensitivity of anti-CCP and anti-MCV antibodies were 67.8% and 78.1% ,and specificity were 89.6% and 94.2% respectively. Conclusion： Anti-CCP and anti-MCV antibodies with good sensitivity and specificity for the diagnosis of the early stage of RA, both can be used as a new markers for diagnostic indicators of the early stage of RA disease.
