冠心病患者血清红细胞体积分布宽度变化的临床意义

摘  要:目的观察血清红细胞体积分布宽度(RDW)与冠心病(CHD)的关系。方法纳入本研究的患者均经冠脉造影检查,CHD组(162例)为确诊CHD患者,对照组(50例)为排除CHD者,CHD组按病变严重程度分为单支、双支和三支组;按临床类型分为稳定型心绞痛(SAP)组和急性冠脉综合征(ACS)组。收集患者年龄、糖尿病、高血压病史及吸烟史,并检测血清RDW、白细胞计数(WBC)、总胆红素(TBIL)、空腹血糖(FPG)、总胆固醇(TC)、甘油三脂(TG)、高密度脂蛋白(HDL)、低密度脂蛋白(LDL)和超敏C反应蛋白(hs-CRP),同时测量收缩压(SBP)和舒张压(DBP)。运用logistic回归分析血清RDW水平与CHD发生的关系;比较各组患者血清RDW水平。结果 CHD组患者血清RDW水平显著高于对照组。Logistic回归分析发现血清RDW水平与CHD发生相关,且随着病变支数增加血清RDW水平升高;但SAP和ACS两组间血清RDW水平差异无统计学意义。结论 CHD患者血清RDW与CHD的发生相关,且能反应冠状动脉病变的严重程度,值得进一步评价。Objective To assess the relationship between red cell volume distribution width(RDW) level and coronary atherosclerosis in patients with coronary heart disease(CHD). Methods According to the results of coronary angiography, 162 patients with CHD were selected to the CHD group. These patients were divided into one-vessel, two- vessel or three-vessel disease group on the basis of the number of diseased coronary branches, and they were also divided into stable angina pectoris(SAP) group and acute coronary syndrome(ACS) group according to the clinical types. A total of 50 patients without CHD were as control group. The level of RDW, white blood cell(WBC) count, total bilirubin(TBIL), fasting plasma glucose(FPG), total cholesterol(TC), triglyceride(TG), high-density lipoprotein(HDL), low- density lipoprotein(LDL), high sensitive C reactive protein(hs-CRP), systolic blood pressure(SBP) and diastolic blood pressure(DBP) were measured, also include age, prior medical histories including hypertension, diabetes mellitus and smoking status. Logistic regression analysis was applied to analyze the association between RDW and CHD. The level of RDW was compared statistically between the groups. Results The level of RDW in CHD patients was significantly higher than that in controls. Logistic regression analysis showed that the level of RDW was related with risk factor of CHD. The level of RDW was increased as the increasing number of diseased coronary branches. No significant difference was found in the level of RDW between patients with ACS and SAP. Conclusion The level of RDW is linked with increased risk of CHD, and related with the severity of coronary atherosclerosis.
