摘 要:目的探讨肾络通拮抗肾间质纤维化的可能机制。方法将48只Wistar大鼠随机分为假手术组、模型组、依普利酮组和肾络通组,每组12只。除假手术组外,其余各组大鼠结扎左侧输尿管造成梗阻性肾病大鼠模型,依普利酮组每天灌胃依普利酮100mg/kg,肾络通组每天灌胃肾络通26g/kg,治疗10天,假手术组及模型组灌胃等体积生理盐水。观察各组大鼠肾脏组织形态学改变,检测血清醛固酮、肿瘤坏死因子(TNF-α)、诱导型一氧化氮合酶(iNOS)、白介素-1β(IL-1β)、血清糖皮质激素诱导蛋白激酶1(SGK-1)及其mRNA表达情况。结果与假手术组比较,模型组大鼠肾脏胶原沉积显著增加,大量炎性细胞浸润;血清醛固酮、TNF-α、iNOS、IL-1β含量显著升高(P<0.05或P<0.01);SGK-1 mRNA及蛋白表达明显增强(P<0.01)。与模型组比较,肾络通组、依普利酮组胶原沉积及炎性细胞浸润均减轻,血清TNF-α、iNOS、IL-1β含量明显减少(P<0.05或P<0.01),SGK-1 mRNA及蛋白水平明显下调(P<0.01);肾络通组血清醛固酮含量亦减少(P<0.01)。结论肾络通能减缓梗阻性肾病模型大鼠肾间质纤维化进程,其可能机制为通过阻断SGK-1信号通路抑制醛固酮活性,从而减轻炎症损伤及炎性介质的表达。Objective To investigate the effects and mechanism of Shenluotong (decoction to free the kidney vessels) on renal interstitial fibrosis. Methods Totally 48 female Wistar rats were randomized into the sham operation group, model group, eplerenone group and Shenluotong group, with 12 in each. The renal interstitial fibrosis models were established by ligation of the left ureter in all groups except the sham operation group after one week of adaptive feeding. The rats in the eplerenone group and Shenluotong group were gavaged with 100 mg/kg eplerenone and 26 g/kg Shenluotong respectively for 10 days. The rats in the sham operation group and model group were gavaged with the same volume of normal saline. The morphological changes and the levels of serum aldosterone, tumor necrosis factor-a ( TNF-c~), induced nitric oxide synthase ( iNOS), interleukin 113 ( IL-113), serum glucoeorticoid-induced protein kinase 1 ( SGK-1 ) mRNA and serum glucocorticoid-induced protein kinase 1 SGK-1 were observed. Results Comparing with the sham operation group, renal pathology showed a significant increment of collagen deposition and inflammatory cell infiltration, the levels of serum aldosterone, TNF-c~, iNOS and IL-1 [3 (P 〈 0. 01 or P 〈 0. 05 ) and the expressions of SGK-1 mRNA and SGK-1 (P 〈0. 01 ) were significantly increased in the model group. Comparing with the model group, the renal collagen deposition, inflammatory cell infiltration, the levels of TNF-et, iNOS and IL-113 (P 〈 0. 01 or P 〈 0. 05 ) and the expressions of SGK-1 mRNA and SGK-1 ( P 〈 0. 01 ) were significantly decreased in the Shenluotong group and eplerenone group (P 〈0. 01 ) and the serum aldosterone level was significantly decreased in the Shenluotong group as well ( P 〈 0. 01 ). Conclusion Shenluotong can delay the process of renal interstitial fibrosis in obstructive nephropathy, which may be associated with inhibiting the activity of aldosterone by blocking SGK-1 signal and reducing inflammatory in
