扶正祛邪法治疗急性呼吸窘迫综合征临床观察

摘　　要：目的:通过对2008—2012年辽宁省中医院重症医学科(Intensive Care Unit,ICU)收治的急性呼吸窘迫综合征(acute respiratory distress syndrome,ARDS)患者进行扶正祛邪中药治疗临床疗效进行观察。明确扶正祛邪法对ARDS患者的作用。方法:根据本研究的病例纳入及排除标准,按患者住院的先后顺序,按照事先由随机数确定的随机分组方法,进入治疗组和对照组。治疗组在呼吸机支持,西药治疗原发病基础上,入组后即开始予中药治疗。并对病死率、脱机率和ARDS评分、APACHEⅡ评分、氧合指数进行统计分析。结果:治疗30天病死率及脱机率比较,与对照组比较治疗组病死率减少、脱机率增加、并能明显较少机械通气时间和住ICU时间,有统计学差异(P≤0.05);且ARDS评分、APACHEⅡ评分与对照组比较治疗组均下降,有统计学差异(P≤0.05),治疗组治疗前后对比,治疗后评分下降,有统计学差异(P≤0.05);与对照组比较治疗组氧和指数有明显改善,有统计学差异(P≤0.05),治疗组前后对比,治疗后氧和指数明显增加,有统计学差异(P≤0.05)。结论:以扶正祛邪法中药治疗ARDS较单纯西医治疗可以明显增加临床疗效。Objective ： To observe the clinical curative effect of Fuzheng Quxie traditional Chinese medicine on acute respiratory distress syndrome （ARDS） patients during 2008 to 2012 in Liaoning Province Hospital of Traditional Chinese Medicine in intensive care medicine （ICU）. To clear Fuzheng Quxie method function on patients with ARDS. Methods： According to this study＇s case criteria and exclusion criteria and the order of inpatients, in accordance with the advance by the random number to determine the stochastic grouping method, patients were divided into the treatment group and control group. Besides ventilator support and Western medicine, patients began to take traditional Chinese medicine treatment af- ter grouping. And the case fatality rate, offline rate and ARDS score, APACHE score, oxygenation index were made statis- tical analysis. Results： 30 day mortality was reduced and offline rate was increased in treatment group compared with those of the control group and it can significantly reduce the duration of mechanical ventilation and ICU time. There was a statistically significant difference between two groups （ P〈0. 05 ） ; and the score of ARDS and APACHE II score in treatment group compared with those in the control group were decreased, and there was statistically significant difference （P〈0.05）. Before and after treatment, after treatment the score was decreased, and there was statistically significant difference （P〈0.05 ）. Compared with the control group, treatment group＇s oxygen index was improved significantly, and there was significant difference （P〈0. 05）. After treatment,the treatment group＇s oxygen index was increased obviously, and there was statistically significant difference （P 〈0. 05 ）. Conclusion： The traditional Chinese medicine in the treatment of ARDS with Fuzheng Quxie method can obviously increase the clinical effects compared with the pure Western medicine treatment.
