冠状动脉病变部位对QT间期离散度的影响

摘　　要：目的 :观察冠状动脉病变对 QT间期及 QT间期离散度 (QTd)的影响。方法 :回顾性分析 5 1例冠心病患者经皮冠状动脉腔内成形术 (PTCA)治疗前 QT间期和 QTd及 PTCA治疗后 QTd缩短情况 ,按冠状动脉病变进行比较。结果 :1PTCA治疗前 QTd在右冠状动脉 (RCA)病变比左前降支动脉 (L AD)病变明显延长 (P<0 .0 1) ;2 PT-CA治疗后 RCA病变 QTd缩短程度比 L AD病变明显 (5 0± 11m s对 12± 10 ms,P<0 .0 1)。 3 PTCA治疗后单支冠状动脉病变 QTd缩短程度比多支冠状动脉病变明显 (2 2± 2 7m s对 5± 17ms,P<0 .0 5 )。结论 :冠状动脉病变使相应部位 QT间期延长 ,QTd增加。 PTCA治疗后 ,QT间期缩短 ,QTd减少。AIM:QT dispersion (QTd) were observed to the effect of diseased coronary vessels site on chronic coronary heart diseases. METHODS:A retrospective study with itself controls, QT interval and QTd changed in 51 patients with chronic coronary heart diseases before and after treatment with percutaneous transluminal coronary angioplasty (PTCA). RESULTS:①Before PTCA therapy,the increase of QTd of diseased right coronary artery (RCA) were significant compared with LAD ( P <0.01)。②Reduction of QTd of RCA were significant after PTCA therapy compared with LAD (50±11 ms vs 12±10 ms, P <0.01)。③Reduction of QTd were significant in patients with 1-vessel diseased of coronary artery after PTCA therapy compared with multiple vessels diseased of coronary artery (22±27 ms vs 5±17 ms, P <0.05). CONCLUSION:The diseased coronary artery prolongs QT interval in correlative region, QTd was increased. After PTCA therapy,QT interval decreased, QTd should be lower relatively. Reduction of QTd is related with the site (number,degree and so on) of the diseased coronary artery.
