组合外固定架治疗成人锁骨中1/3不稳定骨折

摘　　要：目的：探讨应用组合外固定架治疗锁骨中1／3不稳定骨折的方法与疗效。方法：自2008年3月至2011年3月，采用组合式外固定架治疗36例成人锁骨中1／3不稳定骨折，男24例，女12例；年龄18-55岁，平均43岁；左恻20例，右侧16例；受伤至手术时间2-6d，平均3．5d。骨折Robinson分型：2A2型7例，281型18例，282型11例。患者术前均无血管神经损伤。采用Neer肩关节功能评分进行疗效评价。结果：36例患者均获随访，时间6～12个月，平均8个月，均获得骨性愈合。Neer肩关节功能总评分（88．3±6．2）分，其中疼痛（31．6±3．2）分，功能（25．7±2．2）分，活动度（21．1±1．7）分，解剖（8．8±0．8）分；优22例，良13例，可l例。结论：组合外固定架治疗成人锁骨中1／3不稳定骨折可以取得较好疗效，是临床可选择的治疗方法。To explore methods and clinical effects of composite external fixtor in treating adult patients with unstable middle 1/3 of clavicular fractures. Methods：From March 2008 to March 2011,36 patients with unstable middle 1/3 of clavicular fractures were treated with composite external fixtor. There were 24 males and 12 females ,aged from 18 to 55 years old with an average of 43 years old. Twenty eases on the left side and 16 cases on the right side. Time from injury to oper ation was 2 to 6 days （averaged 3.5 days）. According to Robinson classification,there were 7 cases with type 2A2,18 cases with type 2B 1, and 11 cases with type 2B2. No vessels and nerve damage occurred before opreation. The clinical effects were evaluated according to Neer scoring. Results： All cases were followed up from 6 to 12 months with an average of 8 months. The mean Neer score was 88.3±6.2, which included pain 31.6±3.2, functional score 25.7±2.2, range of motion score 21.1 ±1.7, and anatomy score 8.8±0.8. There were 22 cases in excellent ,good in 13 ,fair in 1. Two cases occurred pin tract infection. Conclu sion： Composite external fixtor is an optional method in treating unstable middle 1/3 of clavicular fracture, and can obtain a good clinical effects.
