对比观察MEBO与磺胺嘧啶银治疗耳廓深度烧伤的临床疗效

摘　　要：目的对比观察涂抹湿润烧伤膏(MEBO)后外敷油纱半暴露疗法与涂抹磺胺嘧啶银粉后烤架促进成痂疗法治疗耳廓深度烧伤的临床疗效。方法将105例耳廓烧伤患者随机分为治疗组(56例,共98只耳朵)与时照组(49例,共85只耳朵),治疗组创面采用涂抹MEBO后外敷油纱的半暴露疗法治疗;对照组创面采用涂抹磺胺嘧啶银粉后暴露,并用烤架促进成痂疗法治疗,观察对比两组耳廓烧伤创面的疼痛程度,愈合时间及愈合质量。结果治疗组创面疼痛较轻,平均愈合时间较对照组缩短3.18 d,愈后耳廓色索沉着、脱失及瘢痕、缺损、畸形率均较对照组低。结论耳廓烧伤创面涂抹MEBO后,外敷油纱可最大限度地保留间生态组织,恢复耳廓外形,且患者疼痛较轻,愈合效果良好,值得临床推广应用。Objective To observe and compare the clinical therapeutic effects of MEBO combined with the semi- exposed therapy and Sulfadiazine Silver （SD-Ag） combined with gridiron-promoted scabbing therapy in the treatment of auri- cle deep burns. Methyls A total of 105 patients with auricle burns were randomly divided into a treatment group （56 pa- tients with 98 burned ears） and a control group （49 patients with 85 burned ears）. The treatment group was treated with the MEBO and dressed by MEBO-impregnated gauze, i. e. semi-exposed therapy and the control group was kept exposed after smearing SD-Ag on the wounds and then gridiron-promoted scabbing therapy was used to promote scab formation. The pain degree, healing time and healing quality of the wounds between the two groups were observed and compared. Results Compared with the control group, the pain degree was lower and the mean healing time was 3.18 days shorter in the treat- ment group. The rates of hyperpigmentation, depigmentation, scar formation, defect and deformity of the healed auricles in the treatment group were lower compared with the control groups. Conclusion Topical use of MEBO and dressed by MEBO gauze can remain the viable tissue as much as possible to restore the appearance of auricles, and also can relieve patientsj pain and realize excellent healing effect, which should be promoted and applied in clinic.
