高龄肺炎患者中性粒细胞TLR4表达与临床意义

摘　　要：目的观察高龄肺炎患者中性粒细胞TLR4(Toll like receptors 4,TLR4)表达情况。方法以高龄肺炎患者(肺炎组,n=49)和健康对照者(对照组,n=21)作为研究对象。采用分离外周静脉血中性粒细胞,提取中性粒细胞的mRNA,实时定量-PCR检测TLR4 mRNA表达,比较两组TLR4 mRNA相对表达量。分析肺炎PSI评分(pneumonia severity index,PSI)与TLR4mRNA相对表达量相关性。结果高龄肺炎组中性粒细胞TLR4mRNA相对表达量较对照组中性粒细胞TLR4mRNA相对表达量高(P<0.05),两组间中性粒细胞TLR4mRNA相对表达量比较差异有统计学意义;肺炎PSI评分与中性粒细胞TLR4mRNA相对表达量成正相关(r=0.495,P=0.001)。结论中性粒细胞TLR4信号传导通路可能在高龄肺炎发生发展中起一定作用,为高龄肺炎的免疫治疗奠定了基础。Objective To investigate the expression of toll like receptors 4 （TLR4）in neutrophil of elderly patients with pneumonia. Methods Forty-nine elderly patients with pneumonia （pneumonia group） and 21 normal subjects （control group）were recruited in study. Peripheral venous blood neutrophils were separated and neutrophil mRNA was extracted, TLR4 mRNA expression was detected with real-time PCR. The correlation between pneumonia severity index （PSI）score and TLR4 mRNA relative expression level was analyzed. Results The expression of TLR4mRNA in pneumonia group was higher than that in control group （ P 〈 0.05 ） and pneumonia PSI score was positively correlated with the relative expression of neutrophil TLR4 mRNA （ r = 0. 495, P = 0. 001 ）. Conclusion Neutrophil TLR4 signaling pathway may play a role in the development of elderly pneumonia.
