摘 要:目的探讨人工股骨头置换术后股骨头脱位的发生原因。为提高疗效、减少并发症提供参考。方法回顾性分析自2005—01—2011—12双极人工股骨头置换术治疗216例老年股骨颈骨折的资料。其中33例伴神经系统疾病,包括脑卒中后偏瘫29例、帕金森病4例。术后随访时观察股骨头脱位情况,并进行统计学分析。结果术后均获随访12-72个月,平均35个月。216例中共有9例(4.2%)发生股骨头脱位,其中4例术前伴有神经系统疾病,2例存在假体安装不匹配的问题。伴神经系统疾病患者术后脱位发生率为12.1%(4/33),其他患者术后脱位5例,占2.7%(5/183),两组脱位发生率比较差异有统计学意义(P〈0.05)。结论老年人合并的神经系统疾病与人工股骨头置换术后股骨头脱位具有一定的相关性.在治疗时要加以重视及预防。假体安装匹配不良也是术后发生脱位的重要原因。Objective To explore the causes of dislocation after artificial femoral head replacement, and to provide a reference for improving efficacy and reducing complications. Methods From Jan. 2005 to Dec. 2011, 216 patients treated with artificial femoral head replacement were reviewed retrospectively. There were 33 cases with nervous system disease,4 cases with Parkinson's disease and 29 cases with hemiplegia after stroke. Dislocation of femoral head was observed and statistical analysis was conducted during the follow-up. Results The mean follow-up period was 35 months(range, 12 to 72 months). Dislocation of the femoral head prosthesis was observed in 9 cases (4.2%). There were 4 cases in the 33 patients with nervous system disease, and 5 cases in the other 183 patients, with significant difference between two groups(P 〈0.05). Conclusion Nervous system diseases are related to the dislocation. Preventive measures must be taken to reduce the undesirable side-effects that derived from nervous system disease. Incorrect placement of femoral head prosthesis is also the iinportant reason of dislocation.
