摘  要:目的测定股骨粗隆间骨折患者的血骨代谢生化指标以期预测骨折发生的危险性。方法对142例60岁以上老年股骨粗隆间骨折(骨折组)和141例老年骨质疏松但非骨折患者(对照组)进行血骨代谢测定,测定指标包括Ⅰ型前胶原氨基端延长肽(P1NP)、Ⅰ型胶原羧基端β特殊序列(β-CTX)、骨钙素(OC)、甲状旁腺素(PTH)、25(OH)D3。结果骨折组P1NP、β-CTX、OC和PTH高于对照组,组间差异有统计学意义(P<0.05);两组25(OH)D3差异无统计学意义(P>0.05)。结论老年骨质疏松患者血骨代谢测定中P1NP、PTH、β-CTX和OC增高,可能预测股骨粗隆间骨折风险增高,由此可以识别股骨粗隆间骨折高风险人群并给予及时的抗骨质疏松治疗。Objective To measure the biochemical markers of bone metabolism in the elderly patients with femoral intertrochanteric fracture in order to evaluate the risks of fragile hip fracture. Methods The mean serum levels of procollagen type 1 aminoterminal propeptide (P1NP), cross-linked C-terminal telopeptide of type I collagen (β-CTX),osteocalcin(OC), parathyroid hormone(PTH) and 25 (OH)D3 were measured in 142 patients, over 60 years old with femoral intertrochanteric fracture and 141 elderly patients without fracture history. Results The mean serum levels of P1NP, β-CTX,OC and FrH in fracture group were significantly higher than that in the control greup(P 〈0.05), no difference was found regarding 25(OH)D3 (P 〉0.05). Conclusion Serum P1NP, β-CTX, OC and PTH levels in osteoporetic patients were positively correlated with fragile hip fracture risk, and it is helpful to give those individuals appropriate osteoporosis treatment in time.
