腹腔镜结直肠癌手术并发症的临床分析

摘　　要：目的探讨腹腔镜结直肠癌手术并发症的原因及治疗。方法回顾性分析2009年1月至2013年6月本院59例腹腔镜结直肠癌手术的临床资料。结果 59例患者均痊愈出院,无死亡病例。行直肠前切除术(Dixon手术)25例,乙状结肠切除术14例,腹会阴联合直肠切除术(Miles手术)9例,右半结肠切除术8例,左半结肠切除术3例。57例完成腹腔镜手术,2例(3.4%)中转开腹。术中并发症5例(8.5%),腹腔出血3例,肠破裂2例;术后早期并发症4例(6.8%),2例吻合口瘘,1例小切口感染,1例会阴部切口裂开;1例(1.7%)切口肿瘤种植。结论腹腔镜结直肠癌手术是安全、有效的,术前严格掌握手术适应证,术中精细地操作,将有效的降低手术并发症的发生。Objective To explore the reasons and treatment of complications in laparoscopic surgery for colorectal cancer. Methods The clinical data of 59 patients who underwent laparoscopic surgery for colorectal cancer in our hospital between January 2009 and June 2013 were retrospective analyzed. Results All the 59 patients were recovered, and there was no death.Among the 59 patients,25 underwent anterior resection（Dixon）,14 had sigmoidectomy,9 had abdominoperineal section（Miles）,8 had right hemicolectomy,and 3 had left hemicolectomy.Two patients（3.4%）in the laparoscopic group were converted to the open operation.There were 5 cases（8.5%）of intraoperative complications, including abdominal cavity hemorrhage in 3 cases and intestinal rupture in 2 cases;there were 4 cases（6.8%）of early postoperative complications including anastomotic leakage in 2 cases,small incision infection in 1 case,and perineum incision dehiscence in 1 case;there was 1 case（1.7%）of incision recurrence. Conclusions Laparoscopic surgery is safe and effective for colorectal cancer.To strictly master the operation indications of pre-operation and process accurately during the operations could reduce the incidence of complications effectively.
