牵张训练配合“平衡阴阳”电针法治疗脑卒中痉挛期足内翻的临床观察

摘　　要：目的观察踝关节牵张训练配合"平衡阴阳"电针法治疗脑卒中痉挛期足内翻的疗效。方法 84例患者随机分成研究组与对照组,每组42例,两组均给予脑卒中常规药物治疗及踝关节背屈被动牵张训练,研究组增加"平衡阴阳"电针法进行治疗,两组均连续治疗32 d。采用静息状态足内翻角度的测量、简化的Fugl-Meyer下肢运动功能评定(FMA)和Brunnstrom下肢功能分期评定,对患者足内翻程度和下肢运动功能进行治疗前后评定。结果两组患者经治疗后,静息状态足内翻角度、FMA评分及Brunnstrom下肢功能分期均有明显改善(P<0.01),且治疗后研究组静息状态足内翻角度、FMA评分明显高于对照组(P<0.01),研究组Brunnstrom下肢功能分期与对照组比较有明显改善(P<0.05)。结论踝关节牵张训练配合"平衡阴阳"电针法治疗脑卒中痉挛期足内翻可明显改善足内翻角度,提高患者的下肢运动功能,且疗效优于单纯踝关节牵张训练。Objective To observe the curative effects of ankle stretching exercise combined with ＂Yin-yang balance＂ electro-acupuncture on spasm stage strephenopodia after stroke. Methods 84 patients were randomly divided into observation group and control group with 42 cases in eaeh group. 2 groups were given conventional treatment of stroke and ankle dorsiflexion and passive stretching exercise for 32 days, plus observation group was added ＂Yin-yang balance＂ electro- acupuncture treatment. The strephenopodia and lower limb motor functions were evaluated through testing varus angle at resting state, simplified Fugl-Meyer measurement （FMA） and Brunnstrom stage evaluation of lower extremity function. Results After treatment, the resting state varus angle, FMA score and Brunnstrom stage of lower extremity function were significantly improved in 2 groups （P〈0.01）; the resting state varus angle and FMA score of observation group were better than those of control group （P 〈0.01）; Brunnstrom stage of lower extremity function of observation group was better than that of control group （P〈0.05）. Conclusion Ankle stretching exercise combined with ＂Yin-yang balance＂ electro-acupuncture can significantly improve the varus angle and the lower limb motor function; the curative effects are better than pure ankle stretching exercise.
