记忆合金肋骨环抱接骨器治疗创伤性连枷胸疗效观察

摘　　要：目的探讨应用记忆合金肋骨环抱接骨器治疗创伤性连枷胸的方法和疗效。方法回顾分析我院2006年3月—2012年3月收治的创伤性连枷胸患者69例,其中37例患者采用记忆合金肋骨环抱接骨器手术治疗的做为观察组,其余32例采用加压包扎、牵引外固定等保守治疗方法的做为对照组,进行对比分析,比较2组患者疗效及并发症情况。结果观察组/对照组,疼痛感(3.97±1.07)/(6.49±1.95);胸壁畸形:0/13;平均住院时间(d):(15.32±5.12)/(20.19±6.67);平均机械通气时间(d):(1.69±1.45)/(4.12±2.56);呼吸系统并发症:12/27;死亡:2/3。观察组术后胸壁畸形消失、疼痛明显改善、平均住院时间、平均机械通气时间、呼吸系统并发症(包括肺部炎症、肺不张)的发生率均显著低于对照组(P<0.05),2组病死率差异无统计学意义(P>0.05)。结论记忆合金肋骨环抱接骨器内固定术治疗创伤性连枷胸安全可行,并有疗效高、痛苦小、并发症少等优点。Objective To explore the application and clinical efficacy of memory alloy rib embracing fixators in the treatment of traumatic rib flail chest.Methods A retrospective research was performed from March 2006 to March 2012,and 37 cases of traumatic rib flail chest were treated with the memory alloy embracing fixators as a experimental group and 32 patients were treated with the compression bandaging and traction external fixation methods as a control group.Results The experimental group compared with the control group： the pain：（3.97±1.07）/（6.49±1.95）;chest wall deformities： 0/13;the mean duration of hospitalization（d）：（15.32±5.12）/（20.19±6.67）;the average duration of mechanical ventilation（d）：（1.69±1.45）/（4.12±2.56）;respiratory complications： 12/27;death： 2/3.The postoperative chest wall deformity,pain improvement,the average time of hospitalization,the average duration of mechanical ventilation,and respiratory complications（including pulmonary inflammation and atelectasis） were significantly better or shorter or lower in the experimental group than those in the control group（P0.05）,but there was no significant difference of the mortality between the 2 groups（P0.05）.Conclusions The memory alloy rib embracing fixator internal fixation is effective and safe in the treatment of traumatic flail chest.
