不同止血方法对腹腔镜卵巢囊肿剔除术后卵巢储备功能影响的临床分析

摘　　要：目的:探讨在腹腔镜卵巢囊肿剔除术中采用不同的止血方法对术后卵巢储备功能影响。方法:选择因卵巢囊肿行腹腔镜卵巢囊肿剔除术患者共100例,随机分为PK刀电凝组、缝合止血组各50例;对比两组患者术前、术后1个月、术后3个月时的FSH、LH、E2水平及卵巢大小、窦状卵泡数、月经变化情况、围绝经期症状发生的情况等。结果:术前两组患者的激素水平比较,差异无统计学意义(P>0.05),PK刀组与缝合止血组的术后1个月、术后3个月的FSH及LH值较同组术前相比稍高,差异有统计学意义(P<0.05),但两组患者同期术后FSH及LH值比较差异无统计学意义(P>0.05);两组患者术前的窦状卵泡数与术后比较,差异无统计学意义(P>0.05),PK刀电凝组出现1例围绝经期症状,3例月经改变,缝合组无1例围绝经期症状出现,月经异常仅有1例。结论:PK刀与缝合止血方式均不影响患者术后的卵巢储备功能。Objective： Explore the influence of different hemostatic ways on ovarian function in patients with laparo- scopic ovarian cyst removal surgery. Methods： 100 patients with ovarian cyst who underwent laparoscopic cystectomy were randomly divided into two groups： PK group cautery knife, hemostasis group, 40 cases in each group; FSH, LH, F_~ levels and ovarian size, the number of antral follicles,menstrual change and perimenopausal symptoms occur were de- tected and compared in preoperative and 1 month. And 3 months after operation between the two groups. Results：Pre- operative hormone levels in the two groups were compared, the difference was not statistically significant （P〉0. 05）, the hormone levels of FSH, LH in preoperative groups were compared to postoperative 1,3 months, the difference was statistically significant though the level of the latter higher （P〈0. 05）, but the hormone levels of FSH, LH in corre- sponding time period of two groups were compared, the difference was statistically significant（P〉0. 05）. Two groups of patients before the number of antral follicles after comparison, the difference was not statistically significant （P〉 0. 05）, PK knife appeared one case perimenopausal symptoms, three cases of menstrual changes, suture group no case of perimenopausal symptoms, menstrual abnormalities only one case. Conclusion： Both PK knives and hemostasis meth- ods do not affect postoperative ovarian reserve
