摘  要:目的 通过大鼠腰神经根慢性压迫性损伤模型,探讨神经生长因子(NGF)对腰神经根慢性压迫性损伤致脊髓神经细胞凋亡的影响.方法 将54只大鼠分为生理盐水(NS)对照组、NGF硬膜外隙给药组、NGF尾静脉给药组,术后每组动物分别存活1、2、4周,不同时段各组动物均为6只.手术经大鼠左侧L5椎间孔插入长4 mm、直径0.5~0.6 mm的中空钢管,对神经根形成稳定的压迫,同时中空钢管套接PE管引至项部皮下行硬膜外隙用药.造模术后压迫组与NGF硬膜外隙给药组分别经PE管于硬膜外隙注射NS与NGF,NGF尾静脉给药组经尾静脉注射NGF.术后进行行为学及大体观察,对L5神经根相连的脊髓节段切片进行组织学等染色,观察各组脊髓神经细胞的病理变化.结果 神经根压迫性损伤后,出现神经行为学改变,各时相点NGF给药组神经行为学特征恢复优于对照组(P<0.05);组织学染色证实受压神经根相连的脊髓节段出现神经细胞变性、凋亡,在各时相点NGF给药组与NS对照组比较,NGF给药组凋亡细胞数量减少(P<0.05);NGF给药组Bax蛋白的表达降低(P<0.05);NGF给药组bcl-2蛋白的表达上调(P<0.05),而NGF用药各组间在同一时相点差异无统计学意义(P>0.05).结论 建立大鼠腰神经根慢性压迫性损伤后可致相应脊髓节段神经细胞的凋亡,分别经硬膜外隙或尾静脉给予NGF治疗后均可抑制脊髓神经细胞的凋亡.Objective To investigate the effect of NGF on apoptosis of spinal cord neurons which induced by chronic the injury of compression on lumbar nerve root by the rat models of lumbar nerve root chronic compression injury. Methods 54 SD rats were ran- domly divided into NS control group, NGF epidural space administration group and NGF caudal vein injection group. The animals in each group were separately survived for 1,2 and 4 weeks. There were 6 rats in each group of different time point. During the surgery,the rats" left L5 intervertebral foramen was inserted a 4ram long and 0.5 - 0.6mm in diameter hollow stainless steel tube to set up chronic com- pression in nerve root. The steel tube was socket PE pipe and led to the neck subcutaneous to get NS and NGF administration in epidural space. The NGF caudal vein group was injected NGF via tail vein. After the surgery, the rats'behavior changes were observed. The spinal segment section which was connected with the L5 nerve root was assessed by histological staining. The pathological changes of spinal cord nerve cell were observed. Results After compression injury on the nerve root in rats, neurological damage behavior of the rats were changed. The neurobehavioral characteristics of recovery in the group at each time point treated with NGF was better than that in the con- trol group (P 〈 O. 05). The results of histological staining suggested that nerve cell in the spinal cord segment corresponded with nerve root compression from all of rats appears degeneration and apoptosis. Compared with NS control group,the number of apoptotic cells in the group treated with NGF was reduced at different time point (P 〈 0.05 ), the amount of Bax protein expression was significantly de- creased ( P 〈 0.05 ), the amount of bcl - 2 protein expression was significantly increased ( P 〈0. 05 ). Conclusion The chronic compres- sion injury on rat lumbar nerve root causes neuronal apoptosis of corresponding spinal segment,which are inhibited by the treatment with NGF
