老年维持性血液透析患者血透导管两种封管方法对比研究

摘　　要：目的观察老年维持性血液透析患者血透导管不同封管方法的临床结果。方法将2009—2012年使用血液透析导管作为透析通路的老年透析患者按导管封管液配制方法不同分别设为对照组(尿激酶20 000 U+肝素50 mg)和观察组(尿激酶20 000 U+肝素50 mg+低分子肝素4 100 IU),对2组患者的透析导管堵管率、出血风险及发生原因进行分析。结果对照组的导管堵管率显著高于观察组,差异具有统计学意义(P<0.05)。结论 尿激酶20 000 U+肝素50 mg+低分子肝素4 100 IU肝素封管法可延长老年维持性血液透析患者中心静脉双腔管透析使用寿命,且出血的风险并未增加,能有效降低老年透析患者的导管堵管发生率。Objective To observe the clinical outcomes of different tube sealing methods on sealing of hemodialysis tube in elderly patients with maintenance hemodialysis. Methods The el- derly patients with maintenance hemodialysis collected from 2009 to 2012 were divided into control group （20 000 U ＋ heparin 50 mg urokinase）and observation group（20 000 U ＋ heparin 50 nag urokinase ＋ low molecular heparin 4 100 IU） according to different prepared methods of tube- sealing solution. Plugging rate of hemodialysis tube, hemorrhage risk and causes were analyzed be- tween two groups. Results Plugging rate of hemodialysis tube in the control group was 11.76 %, which was significantly higher than 3.57 % in the observation group, and there was significant dif- ference （P 〈 0.05）. Conclusion Tube - ~aling method of urokinase 20 000 U ＋ heparin 50 mg ＋ low molecular heparin 4 100 IU can prolong the service life of center venous double lumen tube in elderly patients with maintenance hemodialysis and effectively reduce the plugging rate of tube, and meanwhile it not increase hemorrhage risk.
