探讨循证护理干预对青年肺癌患者睡眠障碍的效果

摘　　要：目的探讨青年肺癌患者睡眠障碍的原因及循证护理干预效果。方法将青年肺癌患者70例随机均分为干预组和对照组各35例,对照组采用肿瘤内科护理常规进行护理,干预组在肿瘤内科护理常规基础上,根据睡眠障碍的原因进行针对性循证护理。应用匹兹堡睡眠指数量表(PSQI)对2组患者进行评分。结果精神或心理因素是青年肺癌患者睡眠障碍的主要原因,发生率62.9%(44/70)。循证护理干预后,干预组PSQI总评分与差值及精神负担过重、焦虑或恐惧、抑郁PSQI评分与差值,与对照组比较均P<0.05,睡眠质量显著改善。结论 循证护理干预对青年肺癌睡眠障碍患者的睡眠质量有较好的效果,应成为治疗青年肺癌睡眠障碍的主要方法,特别可成为治疗精神负担过重、焦虑或恐惧及抑郁等所引起睡眠障碍的首要方法。Objective To explore the reason of dyssomnia and effect of evidence-based nursing on treatment of dyssomnia in young patients with lung cancer. Methods 70 young patients with lung cancer were randomly divided into intervention group and control group, 35 cases in each group. The control group was conducted with conventional nursing measures, while the interven- tional group was conducted with reason-targeted evidence-based nursing on the basis of the control group. Scale of Pittsburgh sleep quality index （PSQI） was used to assess the quality of sleep in both groups. Results Mental or psychological factors were the major causes of dyssomnia in young patients with lung cancer, and incidence ratio of these factors was 62.9 % （44/70）; after the im- plementation of evidence-based nursing, there were significant differences of total score of PSQI, heavy mental burden , anxiety or fear and PSQI score of depression between two groups （ P 〈 0.05）. Quality of sleep in the intervention group improved significantly. Conclusion Evidence- based nursing intervention shows a positive efficacy in the treatment of dyssomnia in young patients with lung cancer, which should be suggested as a main therapeutic method for dyssomnia in young patients with lung cancer, especially for the treatment of patients with dyssomnia induced by vari- ous reasons such as heavy mental burden, anxiety or fear and depression.
