摘　　要：目的:探讨更年期高血压女性血压昼夜节律异常与颈动脉内中膜厚度(IMT)和微量白蛋白尿(MAU)的关系。方法:对初发未治疗的更年期高血压女性患者135例进行动态血压测定,按血压昼夜节律划分为杓型(34例)、超杓型(34例)、非杓型(33例)和反杓型(34例)4组,对颈动脉IMT及24h MAU进行测定,并对相关资料进行统计学分析。结果:与杓型组比较,非杓型组、反杓型组IMT[(0.6659±0.1235)mm比(0.9615±0.2368)mm比(1.0138±0.3058)mm]显著增厚(P均<0.01),超杓型组[(0.7238±0.0762)mm]亦显著增厚,P<0.05。与杓型组比较,非杓型组、反杓型组MAU[(27.2285±7.6955)mg/24h比(187.7682±112.9837)mg/24h比(199.2179±147.4490)mg/24h]水平显著升高(P均<0.01)。结论:女性更年期高血压血压昼夜节律异常患者颈动脉内中膜厚度、微量白蛋白尿显著增加,需注意防治。Objective： To explore influence of abnormal circadian rhythm of blood pressure on carotid intima-media thickness （IMT） and microalbuminuria （MAU） in climacteric women with hypertension. Methods： A total of 135 climacteric female patients with untreated primary hypertension received ambulatory blood pressure monitoring （ABPM）. According to circadian rhythm of blood pressure, they were divided into dipper group （n= 34）, over-dipper group （n = 34）, non dipper group （n = 33）, and reverse dipper group （n = 34）. The IMT and 24h MAU of all patients were measured, and relative data received statistical analysis. Results： Compared with dipper group, there were significant increase in IMT [ （0. 6659 ±0. 1235） mm vs. （0. 9615 ＋ 0. 2368） mm vs. （1. 0138 ±0. 3058） mm vs., P〈0. 01 all] in non dipper group, reverse dipper group, there was also significant increase in IMT of over-dipper group [（0. 7238±0.0762） mm], P〈0.05. Compared with dipper group, there were significant increase in MAU level [ （27. 2285 ±7. 6955） mg/24h vs. （187. 7682 ±112. 9837） mg/24h vs. （199. 2179 ±147, 4490） mg/24h, P〈0.01 both] in non dipper group and reverse dipper group. Conclusion： The carotid intima-media thickness and microalbuminuria level significantly increase in climacteric women with hypertension, so it is needed to pay attention to prevention and treatment.
