临床路径管理对社区中青年高血压患者的干预效果

摘　　要：目的：探讨临床路径管理对社区中青年高血压患者的干预效果，为临床路径管理在社区推广提供参考。方法：选择佛山市南海平洲社区486例年龄在30-50岁高血压患者作为研究对象，按数字表法随机分为干预组（240例，给予临床路径管理）和常规管理组（246例，按常规社区防治模式进行管理）。在干预两年后，对比分析两组患者的血压控制水平、服务满意度及36项简明健康调查表（SF-36）生活质量评分情况。结果：两年后，与常规管理组比较，干预组血压显著降低[（145．13±18．56）／（86．15±8．46）mmHg vs．（134．86±16．98）／（80．236．8±2）；服务满意度（56．1％比97．1％）及SF-36生活质量各项评分均显著提高（P均〈0．001）。结论：对社区中青年高血压患者实施临床路径管理效果显著，可使患者养成良好的生活习惯，有效控制血压，同时能显著提高患者的生活质量。Objective： To explore intervention effect of clinical pathway management on young and middle- aged patients with hypertension in community, and provide reference for spread of clinical pathway management in community. Methods： A total of 486 patients with hypertension aged 30-50 years were selected from Pingzhou community of Nanhai district in Foshan city. They were randomly divided into intervention group （n = 240, received clinical pathway management） and routine management group （n = 246, received management of routine community prevention and treatment） according to number table. After two-year intervention, control level of blood pressure, service satisfaction and scores of the 36-Item Short-Form Heath Survey （SF- 36） were compared and analyzed between two groups. Results： Compared with routine management group after two years, there was significant decrease in blood pressure [ （145.13 ±18.56） / （86. 15 ±8.46） mmHg vs. （134.86 ±16. 98） / （80.23 ±6.82） mmHg], and significant increase in service satisfaction （56. 1% vs. 97. 1%） and all scores of SF- 36 in intervention group, P〈0. 001 all. Conclusion： Clinical pathway management is effective for young and middle- aged patients in community. It can help patients develop good living habits, effectively control blood pressure and significantly improve their quality of life.
