环磷酸腺苷葡胺治疗老年舒张性心功能不全的疗效评价

摘　　要：目的通过测定B型尿钠肽(BNP)、6 min步行实验、组织多普勒超声心动图,观察环磷酸腺苷葡胺对老年舒张性心力衰竭(DHF)治疗的有效性。方法将98例老年DHF患者随机分为对照组(48例)与治疗组(50例)。对照组给予常规改善心功能治疗,治疗组在常规治疗上加用环磷酸腺苷葡胺180 mg/次静脉点滴,2次/d,持续14 d。检测和记录治疗前后两组超声心动图的左心室舒张功能指标(Ea、Aa、Ea/Aa)及BNP、6 min步行距离。结果两组治疗后BNP、6 min步行距离、超声心动图指标均较治疗前有改善(P<0.05),且治疗组指标的改善程度优于对照组(P<0.05),住院时间短于对照组(P<0.05)。结论环磷酸腺苷葡胺联合常规的改善心功能治疗可进一步改善老年患者的DHF。Objective To observe the effectiveness of meglumine cyclic adenosine monophosphate on elderly patients with diastolic heart failure （ DHF ） through the detection of type B natriuretie peptide （ BNP ）, six-minute walk test, and Doppler echocardiography. Methods 98 elderly patients with DHF were randomly divided into control（ n = 48 ） and treatment groups （ n = 50）. Both groups received the conventional therapy for the improvement of heart function. In addition, the treatment group received the intravenous drip with meglumine cyclic adenosine monophosphate 180 mg twice per day and continuously for 14 days. Detection and recording were made in the diastolic function indexes of the left ventricle（ Ea, Aa, and Ea/Aa） , BNP, the distance of 6-minute walk in both groups before and after the treatment. Results After the treatment, the BNP, the distance of 6-minute walk, and the indexes of left ventricular diastolic function in both groups were improved （ P 〈 0.05 ）. Furthermore, the improvement in the treatment group was superior to that in the control group（ P 〈 0.05 ）. The length of stay in hospital in the former group was shorter than that in the latter one （ P 〈 0.05 ）. Conclusion Meglumine cyclic adenosine monophosphate combined with the conventional therapy on the improvement of heart function can further improve heart diastolic function in elderly patients.
